目的探讨胚胎发育不良性神经上皮肿瘤(dysembryoplastic neuroepithelial tumor,DNET)的MRI表现,以提高对该病的认识。方法回顾性分析经病理证实的13例DNET的MRI资料,13例均行常规MRI检查,其中10例平扫与增强同时进行,3例仅行常规MRI检查。结果 13例DNET中5例位于颞叶,4例位于额叶,3例位于顶叶,1例位于右侧半卵圆中心。8例为单发,5例为多发。病灶均位于或主要累及皮层,病变以囊性成份为主,单发病灶呈倒三角形或楔形。4例单发病灶内可见多个分隔。MRI表现呈均匀或混杂长T1、长T2信号,Flair序列上病灶周边和分隔呈高信号,DWI扩散不受限呈略低信号,仅1例周围可见水肿及占位效应,余未见明显水肿及占位效应。10例增强扫描患者,4例囊性内分隔及1例囊性内结节强化。结论 DNET的MRI表现有一定的特征性,倒三角形或楔形变以及多发囊变或囊内多发分隔为DNET的特异性表现,结合MRI表现有助于术前确诊。
Objective To investigate the MRI findings of dysembryoplastic neuroepithelial tumor (DNET) in order to improve its understanding of the disease. Methods Thirteen cases of pathologically confirmed DNET were retrospectively analyzed. MRI and MRI were performed in all 13 cases. Of the 13 cases, 10 cases underwent both plain and enhanced MRI and 3 cases underwent routine MRI. Results Among 13 cases of DNET, 5 cases were located in the temporal lobe, 4 cases in the frontal lobe, 3 cases in the parietal lobe, and 1 in the right hemisphere. 8 cases were single and 5 cases were multiple. Lesions are located or mainly involved in the cortex, cystic lesions mainly composed of single lesions were inverted triangle or wedge. Four cases of single lesions visible multiple partitions. MRI showed uniform or mixed long T1, T2 signal, Flair sequence lesions were peripheral and segmental high signal, DWI diffusion was limited to a slightly lower signal, only 1 case of edema and mass effect can be seen around, I did not see significant edema And placeholder effect. Ten patients underwent enhanced scan, 4 patients had cystic internal separation and 1 cystic nodule enhancement. Conclusion The MRI findings of DNET have certain characteristics. The inverted triangles or wedges as well as the specific features of multiple cystic degeneration or intracapsular multiple segregation are DNET. Combined with MRI, it is helpful for preoperative diagnosis.
