目的探讨血清可溶性转铁蛋白受体(soluble transferrin receptor,s-TfR)水平在慢性肾脏病(chronic kidney disease,CKD)晚期肾性贫血患者中的临床意义。方法 CKD 4~5期患者60例,测定血清s-TfR、铁蛋白、铁、转铁蛋白等指标水平,计算肾小球滤过率和转铁蛋白饱和度,比较CKD 4期与5期患者铁状态评估指标的差异,分析s-TfR与铁蛋白及转铁蛋白饱和度的相关性。结果 CKD 4期患者铁蛋白、转铁蛋白饱和度明显高于5期患者,而s-TfR明显低于5期患者(P均<0.05);s-TfR与铁蛋白及转铁蛋白饱和度呈负相关(r=-0.398,P<0.05;r=-0.817,P<0.01)。结论 CKD5期患者铁缺乏状态较CKD 4期患者严重,s-TfR可作为判断CKD晚期肾性贫血患者体内铁缺乏的指标之一。
Objective To investigate the clinical significance of serum soluble transferrin receptor (s-TfR) levels in patients with advanced renal anemia in patients with chronic kidney disease (CKD). Methods Sixty patients with CKD stage 4 ~ 5 were enrolled in this study. Serum levels of s-TfR, ferritin, iron and transferrin were measured to calculate glomerular filtration rate and transferrin saturation. Iron status assessment of the differences, analysis of s-TfR and ferritin and transferrin saturation correlation. Results The serum levels of ferritin and transferrin in patients with CKD stage 4 were significantly higher than those in stage 5 and s-TfR was significantly lower than those in stage 5 (all P <0.05). The s-TfR and ferritin and transferrin saturation Negative correlation (r = -0.398, P <0.05; r = -0.817, P <0.01). Conclusions The iron deficiency status in patients with CKD stage 5 is more serious than that in stage 4 CKD patients, and s-TfR can be used as one of the indicators for determining iron deficiency in patients with advanced CKD.
