目的分析导致经外周静脉置入中心静脉导管(PICC)首次穿刺失败的原因并提出应对措施,以降低其穿刺失败率。方法回顾性分析2015年1月-3月门诊就诊的335例PICC患者中出现首次穿刺失败的原因。结果 335例患者中,首次穿刺失败55例,首次穿刺失败率为16.42%。二分类logistic回归分析得出性别、血管部位和血管直径对穿刺失败与否有影响(P<0.05)。结论性别、血管部位和血管直径是穿刺失败与否的独立影响因素。临床置管操作工作中应考虑上述因素,以降低穿刺失败率。
Objective To analyze the causes of the failure of the first puncture of peripherally inserted central venous catheter (PICC) and to put forward countermeasures to reduce the failure rate of puncture. Methods Retrospective analysis of the reasons for the first failed puncture in 335 PICC patients from January to March 2015 in outpatient clinic. Results Among 335 patients, the first puncture failed in 55 cases, the first puncture failure rate was 16.42%. The dichotomous logistic regression analysis showed that gender, location of blood vessel and diameter of blood vessel had no effect on the failure of puncture (P <0.05). Conclusion Gender, location of blood vessel and blood vessel diameter are independent influencing factors of failed or not puncture. Clinical catheter handling should consider the above factors in order to reduce the puncture failure rate.
