目的:观察肝硬化患者不同肝功能级别与心电图QT改变之间的关系。方法:选取本院2014年3月至2015年3月接诊的肝硬化患者120例,年龄51.7±10.0岁,按照CHild-Pugh肝功能分级标准分为CHild-Pugh1组40例,CHild-Pugh2组40例,CHild-Pugh3组40例,另外选取同期健康体检者120例作为对照,年龄48.3±9.4岁。分别对患者进行心电图检查,测量并记录QT间期,心率计算矫正的QT期间(QTcd)等于最大校正QT间期(QTc)减去最小校正QT间期(QTc)。比较每组患者的心率计算矫正的QT期间(QTcd)的平均值以及延长患者之间的比例。结果:CHild-Pugh1组37.5±4.8、CHild-Pugh2组38.8±4.7、CHild-Pugh3组46.7±7.1、健康组53.7±8.0,1组和健康组无QTcd延长≥50ms者,2组和3组的延长比率分分别为25%和55%,1组与健康组患者的数据比较无明显差异,2组和3组的比较差异显著(P<0.05),具有统计学意义。此外,3组较1组、2组的QTcd进一步增大,延长时间比例增加。结论:肝硬化患者的心电图检查存在多种异常,而QTc延长与肝硬化的严重程度有关。
Objective: To observe the relationship between different liver function levels and QT changes in patients with cirrhosis. Methods: A total of 120 patients with cirrhosis admitted from March 2014 to March 2015 in our hospital were enrolled in this study. The patients were 51.7 ± 10.0 years old. According to the CHild-Pugh classification of liver function, they were divided into CHild-Pugh1 group (40 cases), CHild-Pugh2 group 40 cases, CHild-Pugh3 group of 40 cases, another 120 healthy subjects were selected as control, age 48.3 ± 9.4 years. The electrocardiogram (ECG) was performed on each patient. The QT interval was measured and recorded. The corrected QT period (QTcd) of heart rate was equal to the QTc (QTc) minus the QTc (QTc). The heart rate was compared for each group to calculate the mean of the corrected QT period (QTcd) and to extend the proportion of patients. Results: There was no prolongation of QTCD≥50ms in CHild-Pugh1 group, 38.8 ± 4.7 in CHild-Pugh2 group, 46.7 ± 7.1 in CHild-Pugh3 group, 53.7 ± 8.0 in healthy group and ≥50ms in healthy group, The extension ratio was 25% and 55% respectively. There was no significant difference between the two groups and the healthy group (P <0.05), which was statistically significant. In addition, 3 groups than 1 group, 2 groups of QTcd further increased, the proportion of extended time increased. Conclusion: ECG abnormalities in patients with liver cirrhosis have many abnormalities, and the prolongation of QTc is related to the severity of liver cirrhosis.
