浙江省宁波市江东区自鹤街道社区卫生服务中心2011年9月起开展家庭医生服务。是宁波市首批试点单位,经过1年多的实践,取得了一定的成效和经验,初步构建了以责任医生团队为依托的家庭医生服务网络,开展了以健康管理为基石的家庭医生服务内涵建设,实行了以“有效工时”为核心的绩效考评体系,建立了以远程挂号、远程心电影像会诊系统和移动工作包为基础的信息化平台,为全面推进家庭医生服务工作奠定了扎实的基础。
