目的 采用二维斑点追踪(2D-STE)技术评价系统性红斑狼疮(SLE)患者的右心房功能变化.方法 选取A组(对照组)为健康志愿者30例.根据患者肺动脉收缩压(PASP)不同的SLE患者102例分为3组(实验组),B组PASP≤30 mm Hg,C组30 mm Hg<PASP<50 mm Hg,D组PASP≥50 mm Hg.采用常规二维超声测量心室收缩期末右心房的最大容积、最小容积及右心房的收缩前容积(RAVmax、RAVmin及RAVpre),计算右心房的被动排空分数及其主动排空分数(RAPEF及RAAEF).2D-STE技术获取收缩期右心房整体峰值应变率、舒张早期右心房整体峰值应变率及舒张晚期右心房整体峰值应变率(SRs、SRe及SRa).结果 B组所有测量的结果同A组相比无统计学意义(均P>0.05).C组中RAVmax、RAVmin、RAVpre、RAAEF和SRa与A、B组相应值比较均增大(均P<0.05),RAPEF、SRe和SRs均减小(均P<0.05).D组中RAVmax、RAVmin、RAVpre、RAAEF和SRa与A、B、C组比较明显增大(均P<0.05),RAPEF、SRe和SRs明显减小(均P<0.05).结论 2D-STE可以评价不同程度PASP的SLE患者的右心房的功能.“,”Objective To assess right atrial function patients with systemic lupus erythematosus(SLE)by two-dimensional speckle tracking echocardiography(2D-STE).Methods Thirty matched healthy adults selected as the control group A.In addition,102 patients with SLE were divided into three groups according to the difference of pulmo nary artery systolic pressure (PASP),group B was PASP≤30 mm Hg,30 mm Hg <PASP<50 mm Hg as group C,group D was PASP≥50 mm Hg.Right atrial maximum volume(RAVmax),Right atrial minimum volume(RAVmin) and right atrial pre-atrial contraction volume (RAVp) were measuring by conventional echocardiographic,then,calculated right atrial passive ejection fraction (RAPEF) and active ejection fraction (RAAEF).Global peak systolic strain rate,early and late diastolic strain rate (SRs,SRe and SRa) of right atrium were all obtained by 2D-STE.Results RAVmax,RAVmin,RAVp,RAAEF and SRa were significantly increased in group C compared with group A and B,and these parameters in group D were significantly higher than other three groups (all P<0.05).While RAPEF,SRe and SRs were significantly decreased in group C compared with group A and B,and also there parameters in group D were significantly lower than other three groups (all P<0.05).Conclusions 2D-STE could effectively assess the right atrial function of SLE patients with different PASP.
