[目的]探讨染色体13q31.1-rs1359790多态对2型糖尿病患者血糖控制水平的影响。[方法]2006年10月采用随机抽样的方法调查浦东新区934例2型糖尿病患者,2011年9—10月采用重复调查研究的方法追踪随访508例。通过问卷调查、体格检查和生化检测,了解研究对象的一般特征、膳食摄入、生理指标和血糖控制水平;采用Taqman等位基因分型试验对13q31.1-rs1359790位点进行基因分型;广义线性模型分析rs1359790位点多态与血糖控制水平的关联。[结果]所研究人群的rs1359790G/A基因型频率为AA 0.078、AG 0.404、GG 0.518,分布符合Hardy-Weinberg平衡(P=0.971)。广义线性模型分析显示,在调整可能混杂因素后,rs1359790基因型与两次调查间糖化血红蛋白下降幅度的关联有统计学意义(P=0.031),该位点的风险等位基因G可降低糖化血红蛋白水平的下降幅度。膳食纤维摄入与染色体13q31.1-rs1359790多态对2型糖尿病患者血糖控制存在负向交互作用(P=0.016),在低膳食纤维摄入组中,AA+AG基因型患者糖化血红蛋白水平的下降幅度高于GG基因型患者(P=0.002)。[结论]染色体13q31.1-rs1359790多态对2型糖尿病患者血糖控制有影响,且膳食纤维摄入对2型糖尿病患者血糖控制的影响受该多态修饰。
[Objective] To investigate the effect of chromosome 13q31.1-rs1359790 polymorphism on blood glucose control in type 2 diabetic patients. [Methods] A total of 934 type 2 diabetic patients in Pudong New Area were investigated by random sampling in October 2006 and 508 cases were followed up from September to October in 2011 by repeated survey. Through questionnaire, physical examination and biochemical tests, the general characteristics, dietary intakes, physiological indexes and blood glucose control level of the subjects were studied. The 13q31.1-rs1359790 locus was genotyped by Taqman allelic typing test. Linear model analysis rs1359790 locus polymorphism and blood glucose control level. [Results] The frequency of rs1359790G / A genotype in the study population was AA 0.078, AG 0.404, GG 0.518, and the distribution was in accordance with Hardy-Weinberg equilibrium (P = 0.971). The generalized linear model analysis showed that rs1359790 genotype was significantly associated with the decrease of HbA1c between the two surveys (P = 0.031) after adjustment for possible confounding factors. The risk allele G at this site could reduce HbA1c The level of decline. There was a negative interaction between dietary fiber intake and chromosome 13q31.1-rs1359790 polymorphism on glycemic control in patients with type 2 diabetes (P = 0.016). In the low dietary fiber intake group, the level of HbA1c in patients with AA + AG genotype Decreased more than GG genotype patients (P = 0.002). [Conclusion] Chromosome 13q31.1-rs1359790 polymorphism has an effect on the blood sugar control in patients with type 2 diabetes, and the effect of dietary fiber intake on glycemic control in patients with type 2 diabetes is modified by this polymorphism.
