目的:应用CT灌注成像研究肝炎后肝硬化演变过程中的原发性肝癌血流动力学变化规律。方法:肝炎肝纤维化伴原发性肝癌或肝炎后肝硬化伴原发性肝癌患者32例,进行同层动态扫描,测量出肝脏实质与原发性肝细胞癌实体部分的BF、BV、MTT、PS、HAF、IRFTO等灌注指标,每个指标均应用配对t检验,分别对肝脏实质与原发性肝癌进行统计学比较。结果:对比于肝脏实质,原发性肝癌的BF、BV、PS、HAF值升高,MTT、IRFTO值则下降,全部指标均P<0.05,有统计学意义。结论:在肝炎后肝硬化演变过程中,原发性肝癌有着不同于其本底基础的显著的血流动力学变化。
Objective: To study the changes of hemodynamics of primary liver cancer during the development of liver cirrhosis by CT perfusion imaging. Methods: Thirty-two patients with hepatic fibrosis with primary hepatocellular carcinoma or with posthepatitic cirrhosis and primary hepatocellular carcinoma underwent dynamic scans at the same level to measure the BF, BV and MTT of the liver parenchyma and the primary hepatocellular carcinoma , PS, HAF, IRFTO and other perfusion indicators, each of the indicators are used paired t test, the liver were compared with the primary liver cancer statistics. Results: Compared with the liver parenchyma, the values of BF, BV, PS and HAF in primary hepatocellular carcinoma increased, while those in MTT and IRFTO decreased, all P <0.05, with statistical significance. CONCLUSIONS: Primary liver cancer has significant hemodynamic changes that differ from its basal background during the development of posthepatitic cirrhosis.
