目的探讨乌鲁木齐市冬季严重颅脑损伤患者的急救与预后以及患者血糖、血胰岛素水平与预后的关系。方法分析乌鲁木齐市2006年3月~2010年3月间冬季收治的62例重型颅脑损伤的入院急救和疗效,对照组随机选择同年非冬季受伤患者。结果冬季组患者的死亡率显著高于对照组,而入院治疗时间显著长于对照组(均P﹤0.05)。结论冬季重型颅脑外伤患者的急救关键是及早降低颅内压,改善脑组织的微循环;积极手术以及手术后的其他综合治疗,有助于改善预后,减少残障及死亡率。
Objective To investigate the emergency treatment and prognosis of patients with severe head injury in winter in Urumqi and the relationship between the level of blood glucose, blood insulin and prognosis in patients with severe head injury in winter in Urumqi. Methods A total of 62 cases of severe craniocerebral injury admitted to Urumqi from March 2006 to March 2010 were enrolled in this study. The control group were randomized to receive non-winter injury in the same year. Results The mortality rate in winter group was significantly higher than that in control group, while the treatment time in hospitalization was significantly longer than that in control group (all P <0.05). Conclusion The key point of emergency treatment for patients with severe craniocerebral trauma in winter is to reduce intracranial pressure as early as possible to improve the microcirculation of brain tissue. Active surgery and other comprehensive treatment after surgery may help to improve prognosis and reduce disability and mortality.
