目的 :研究根治性颈淋巴清扫术对头颈部血流动力学的影响。方法 :选用 4只成年猕猴 ,模拟人类双侧颈淋巴清扫术 ,作颈部静脉处理 ,观察颈部各重要血管的血流量和压力变化。结果 :颈部静脉的阻断会引起颈部静脉血流动力学指标的明显改变 ,被阻断的颈静脉越多 ,对颈部静脉的血流动力学指标干扰就越大 ;但对颈动脉的相关指标影响甚微。结论 :在根治性颈淋巴清扫术中 ,应尽可能保存颅内静脉回流通道以减少对颈部血流动力学的干扰。其中保留颈外静脉的术式是减少这一干扰的简单而有效的方法。
Objective: To study the effect of radical neck dissection on head and neck hemodynamics. Methods: Four adult macaques were selected to simulate bilateral cervical lymph node dissection. The neck vein was used to observe the changes of blood flow and pressure in the neck. Results: Blocking of the cervical vein caused a significant change of the hemodynamics of the neck vein. The more the jugular vein was blocked, the more interference the hemodynamics of the neck vein was. However, the carotid artery The relevant indicators have little effect. CONCLUSION: In radical neck dissection, intracranial venous return channels should be preserved as much as possible to reduce neck hemodynamic disturbances. One of the surgical procedures to preserve the external jugular vein is a simple and effective way to reduce this interference.
