目的评价上颌前牙区接受单颗种植修复患者应用不翻瓣微创种植的效果。方法 60例上颌前牙区接受单颗种植修复患者,按照随机数字法分为对照组和观察组,各30例。对照组患者给予翻瓣种植,观察组患者给予不翻瓣种植。在种植体愈合6个月以后进行冠修复。在手术后6、12个月时对患者进行锥形束CT影像学检查,同时对种植体唇舌侧骨量变化情况进行评价。在完成冠修复6、12个月对种植体牙周探诊深度、龈乳头指数、改良龈沟出血指数进行检测,并使用红色美学评分对种植体进行美学评价。结果观察组在6、12个月时唇侧骨吸收量和舌侧骨吸收量均少于对照组,差异有统计学意义(P<0.05)。观察组患者改良龈沟出血指数在冠修复即刻时高于对照组,差异有统计学意义(P<0.05)。观察组患者龈乳头指数在完成冠修复6个月时高于对照组,差异有统计学意义(P<0.05)。观察组在6、12个月时红色美学评分分别为(9.60±1.96)、(10.10±2.05)分,均高于对照组的(8.20±1.56)、(9.10±1.65)分,差异有统计学意义(P<0.05)。结论在上颌前牙区接受单颗种植修复患者中应用不翻瓣种植手术,既可促进周牙龈乳头的成形,又能使唇舌侧骨吸收量减少,具有临床美学效果,可广泛推广于临床。
Objective To evaluate the effect of non-flap minimally invasive surgery in patients with maxillary anterior teeth receiving single implants. Methods Sixty cases of maxillary anterior teeth receiving single implants were divided into control group and observation group according to random number method, 30 cases in each group. Patients in the control group were treated with flaps, while patients in the observation group were treated with flaps. Crown restoration was performed 6 months after the implant was healed. Conical beam CT imaging was performed at 6 and 12 months after surgery, and bone morphological changes of the lingual and lingual implants were evaluated. At 6 and 12 months after crown restoration, the periodontal exploration depth, gingival papillae index and modified gingival sulcus bleeding index were measured. The aesthetics of the implants was evaluated using the red aesthetic score. Results In the observation group at 6 and 12 months, the labial bone absorption and lingual bone resorption were less than those in the control group, with statistical significance (P <0.05). In the observation group, the improved sulcus bleeding index was significantly higher than that of the control group immediately after the crown was repaired (P <0.05). The gingival papillae index in the observation group was higher than that in the control group at 6 months after the crown was repaired (P <0.05). The red aesthetic scores of the observation group at 6 and 12 months were (9.60 ± 1.96) and (10.10 ± 2.05) points, respectively, which were significantly higher than those in the control group (8.20 ± 1.56 and 9.10 ± 1.65, respectively) Significance (P <0.05). Conclusion The application of non-flap implant surgery in patients with single anterior maxillary anterior teeth implantation can not only promote the formation of papilla of the peri-lingual papilla, but also reduce the amount of bone absorption on the lingual and lingual sides. It has clinical aesthetics and can be widely used in clinical practice.
