目的:对比研究CT与MRI对不同类型外伤性寰枢椎骨折的诊断价值。方法:回顾我院70例高度怀疑外伤性寰枢椎骨折患者的CT及MRI影像资料,评估CT与MRI检查对外伤性寰枢椎骨折中不同骨折类型的检出结果及显示情况。结果:在70例患者中CT发现寰枢椎骨折61例,其中屈曲型34例,伸展型9例,垂直型12例,旋转侧屈型6例;MRI发现寰枢椎骨折58例,其中屈曲型36例,伸展型3例,垂直型11例,旋转侧屈型8例。结论:对外伤性寰枢椎骨折的总体检出率CT高于MRI,不同受力机制的外伤性寰枢椎骨折CT与MRI的检出率存在着较大的差异。伸展型骨折CT的检出率明显高于MRI,屈曲型骨折和旋转侧屈型骨折MRI的检出率略高于CT,垂直型骨折CT与MRI的检出率无明显差异。
Objective: To compare the diagnostic value of CT and MRI in different types of atlantoaxial fractures. Methods: The CT and MRI images of 70 patients with highly suspected traumatic atlantoaxial fractures in our hospital were retrospectively reviewed. The results of CT and MRI examinations of different fracture types in traumatic atlantoaxial fractures were evaluated. Results: Of the 70 patients, 61 cases of atlantoaxial fractures were found in CT, including 34 cases of flexion, 9 cases of extension, 12 cases of vertical and 6 cases of rotation flexion. MRI showed 58 cases of atlantoaxial fractures, including flexion 36 cases, 3 cases of extension, 11 cases of vertical type, 8 cases of lateral rotation flexion. Conclusion: The overall detection rate of traumatic atlantoaxial fractures is higher than that of MRI. There is a big difference between CT and MRI in traumatic atlantoaxial fractures with different stress mechanisms. The detection rate of CT in stretching fractures was significantly higher than that of MRI. The detection rate of MRI in flexion fractures and rotational flexion fractures was slightly higher than that in CT. There was no significant difference in the detection rate of CT and MRI in vertical fractures.
