目的探讨氧化应激(OS)与T2DM的关系,以及对IR、胰岛素β细胞的影响。方法将FPG≥10mmol/L、未使用过口服降糖药物或胰岛素治疗的新诊断T2DM患者60例作为试验(T2DM)组,将门诊查体的健康者60名作为对照(NC)组。检测FPG、2hPG、糖化血清白蛋白(GA)、TG、LDL-C、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、丙二醛(MDA)水平,计算胰岛素抵抗指数(HOMA-IR)及胰岛素分泌指数(HOMA-β)。结果 (1)T2DM组FPG、2hPG、GA、TG、LDL-C均较NC组升高(P<0.01)。(2)T2DM组SOD、GSH-Px,以及HOMA-β均较NC组降低,OS指标MDA及HOMA-IR较NC组高(P<0.01)。(3)Pearson相关分析表明,T2DM患者FPG与MDA水平呈正相关(r=0.944,P<0.01),GA与MDA水平呈正相关(r=0.392,P<0.05),HOMA-IR与FPG、GA、MDA水平呈正相关(P<0.01),HOMA-β与FPG、2hPG、MDA水平呈负相关(P<0.01)。(4)Logistic回归分析显示,SOD、MDA是T2DM的影响因素。结论 T2DM患者存在糖脂代谢异常及OS。OS对胰岛β细胞分泌功能及IR有影响。
Objective To investigate the relationship between oxidative stress (OS) and T2DM and its effect on IR and insulin β cells. Methods Sixty new cases of T2DM with FPG≥10mmol / L, untreated oral hypoglycemic drugs or insulin were selected as the experimental group (T2DM) and 60 healthy subjects as the control group (NC). The levels of FPG, 2hPG, GA, TG, LDL-C, SOD, GSH-Px and MDA were measured. The insulin resistance index (HOMA-IR) and insulin secretion index (HOMA-β) were calculated. Results (1) FPG, 2hPG, GA, TG and LDL-C in T2DM group were significantly higher than those in NC group (P <0.01). (2) The levels of SOD, GSH-Px, HOMA-β in T2DM group were lower than those in NC group, while the MDA and HOMA-IR indexes in OS group were higher than those in NC group (P <0.01). (3) Pearson correlation analysis showed that there was a positive correlation between FPG and MDA level in T2DM patients (r = 0.944, P <0.01), GA and MDA levels (r = 0.392, MDA level was positively correlated (P <0.01). HOMA-β was negatively correlated with FPG, 2hPG and MDA levels (P <0.01). (4) Logistic regression analysis showed that SOD and MDA were the influencing factors of T2DM. Conclusion T2DM patients have abnormal glucose and lipid metabolism and OS. OS has an effect on pancreatic β-cell secretion and IR.
