目的探讨免疫增强剂甘露聚糖肽注射液对用腹腔镜手术治疗的急性盆腔炎患者的临床疗效和免疫功能的影响。方法 60例急性盆腔炎抗感染治疗疗效差行腹腔镜治疗的患者随机分为治疗组和对照组,每组各30例,对照组给予环丙沙星、甲硝唑等抗感染药物治疗;治疗组在对照组基础上给予甘露聚糖肽注射液10 mg/d静脉滴注。术后10 d根据症状及体征评价临床疗效,根据T细胞亚群及NK细胞比率的变化评价对免疫功能的影响,并随访1年。计量资料进行组间比较时采用t检验,计数资料进行率的比较时采用χ2检验,P<0.05为差异具有统计学意义。结果治疗组总有效率96.67%(29/30),虽略高于对照组的90.00%(27/30),但2组比较差异无统计学意义(χ2=2.706,P>0.05)。治疗组T细胞亚群中的CD3+、CD4+比率,CD4+/CD8+比值,NK细胞比率,均较对照组显著提高(P<0.05)。随访1年,治疗组无复发病例,对照组有2例复发,差异有统计学意义(P<0.05)。结论腹腔镜手术治疗急性盆腔炎加用甘露聚糖肽能提高患者免疫功能,减少术后复发。
Objective To investigate the effect of immunostimulant mannan polysaccharide injection on the clinical efficacy and immune function in patients with acute pelvic inflammatory disease treated by laparoscopy. Methods Sixty patients with acute pelvic inflammatory disease were divided randomly into treatment group and control group, with 30 cases in each group. The control group was treated with anti-infective drugs such as ciprofloxacin and metronidazole. Group on the basis of the control group given mannan peptide injection 10 mg / d intravenous infusion. On the 10th day after operation, the clinical efficacy was evaluated according to the symptoms and signs. The effects on the immune function were evaluated according to the changes of T cell subsets and NK cell ratio and followed up for 1 year. Measurement data for the comparison between groups using t test, count data for the comparison of rates using χ2 test, P <0.05 for the difference was statistically significant. Results The total effective rate in the treatment group was 96.67% (29/30), which was slightly higher than that in the control group (90.00%, 27/30). There was no significant difference between the two groups (χ2 = 2.706, P> 0.05). The CD3 +, CD4 + ratio, CD4 + / CD8 + ratio and NK cell ratio in the T cell subsets in the treatment group were significantly higher than those in the control group (P <0.05). Follow-up 1 year, the treatment group without recurrence, the control group had 2 cases of recurrence, the difference was statistically significant (P <0.05). Conclusion Laparoscopic surgery for the treatment of acute pelvic inflammatory disease plus mannan peptide can improve immune function and reduce postoperative recurrence.
