背景:伤科消炎膏能够缓解膝关节骨性关节炎患者膝关节滑膜炎症,显著改善膝关节肿胀和疼痛,但对于其作用机制目前尚缺乏研究。作者通过动物实验观察其对膝关节滑膜中白介素-1(IL-1)、白三烯(LT)、金属蛋白酶-3(MMP-3)浓度的影响以期初步探讨其作用机制。目的:观察伤科消炎膏对木瓜蛋白酶诱导兔膝骨关节炎性模型的膝关节滑膜中IL-1、LT、MMP-3浓度的影响。方法:通过木瓜蛋白酶关节腔内注射建立18个兔右膝骨关节炎性模型,然后将其随机分为空白组、伤科消炎膏组和双氯芬酸二乙胺组,每组6只。空白组不进行治疗,伤科消炎膏组外用伤科消炎膏治疗、双氯芬酸二乙胺组外用双氯芬酸二乙胺治疗。于治疗后第11天用ELISA法检测试验动物右膝关节滑膜组织中IL-1、LT、MMP-3的浓度。结果与结论:空白组滑膜IL-1浓度高于伤科消炎膏组(P<0.05),空白组滑膜LT浓度高于双氯芬酸二乙胺组(P<0.05),各组间滑膜MMP-3浓度差异无显著性。本研究结果提示伤科消炎膏系通过降低滑膜组织中部分与炎性反应相关的细胞因子水平来缓解膝关节骨性关节炎的滑膜组织炎性反应,其作用机制与双氯芬酸二乙胺类似,但作用途径与双氯芬酸二乙胺相比有所不同。伤科消炎膏对炎性滑膜组织细胞因子中的IL-1具有影响,而双氯芬酸二乙胺对炎性滑膜组织细胞因子中的LT具有影响。但实验结果未显示伤科消炎膏对于滑膜MMP-3浓度有降低作用,说明该药物在抑制滑膜组织胶原降解方面作用不显著。
BACKGROUND: Injury and anti-inflammatory cream can relieve knee joint synovitis in patients with knee osteoarthritis and significantly improve knee joint swelling and pain. However, there is currently no research on its mechanism of action. The effects of interleukin-1 (IL-1), leukotrienes (LT) and metalloproteinase-3 (MMP-3) in synovium of knee joint were observed by animal experiments in order to investigate its mechanism. Objective: To observe the effect of Shangke Xiaoyan Ointment on the concentration of IL-1, LT and MMP-3 in synovium of knee joint induced by papain in rabbit knee osteoarthritis model. Methods: Eighteen rabbit models of right knee osteoarthritis were established by intra-articular injection of papain. The rabbits were randomly divided into blank group, Shangke Xiaoyan cream group and diclofenac diethylamine group, 6 in each group. In the blank group, no treatment was performed. The traumatic anti-inflammatory cream group was treated with Shangke Xiaoyan Cream, and the diclofenac diethylamine group was treated with diclofenac diethylamine. The concentrations of IL-1, LT and MMP-3 in the synovial tissue of the right knee joint were detected by ELISA method on the 11th day after treatment. RESULTS AND CONCLUSION: The concentration of IL-1 in synovial membrane of blank group was higher than that in Shangke Xiaoyan Ointment group (P <0.05). The synovial membrane LT concentration of blank group was higher than that of diclofenac diethylamine group (P <0.05) -3 concentration difference was not significant. The results of this study suggest that Shangke Xiaoyan Cream can relieve the inflammatory reaction of synovial tissue of knee osteoarthritis by decreasing the level of cytokines involved in inflammatory reaction in synovial tissue. The mechanism of action is similar to that of diclofenac diethylamine , But the route of action is different from diclofenac diethylamine. Injury anti-inflammatory cream has an effect on IL-1 in inflammatory synovial tissue cytokines, while diclofenac diethylamine affects LT in inflammatory synovial tissue cytokines. However, the experimental results did not show that the KFC anti-inflammatory cream has a decreasing effect on synovial MMP-3 concentration, indicating that the drug has no significant effect on inhibiting synovial tissue collagen degradation.
