目的探讨脑出血灶周组织及血清肿瘤坏死因子α(TNFα)含量、灶周白细胞浸润与神经元凋亡的关系。方法健康家犬30只,在数字减影全脑血管造影(DSA)介入下刺破大脑中动脉制作基底节区或颞叶脑出血模型,并分为出血后6h、12h、24h、48h、72h及96h组,每组5只。每只犬于制模前及相应时间处死前分别检测血清TNFα含量;处死后取脑检测出血灶周TNFα含量、计数浸润的白细胞及凋亡的神经元,并与健康对照组(5只)进行比较。结果模型犬脑出血灶周及血清TNFα含量分别在出血后24h和48h达峰值,两者呈正相关(r=0.914,P<0.05);出血灶周神经元凋亡及白细胞浸润均于48h达高峰,两者呈正相关(r=0.971,P<0.05),且均与灶周TNFα含量呈正相关(r分别为0.949和0.801,均P<0.05)。结论TNFα及白细胞浸润参与了脑出血后脑组织的损伤过程,两者可能与神经元凋亡有关。
Objective To explore the relationship between perihematoma tissue and serum levels of tumor necrosis factor α (TNFα), peritumoral leukocyte infiltration and neuronal apoptosis. Methods Thirty healthy dogs were punctured through the middle cerebral artery (DSA) to establish basal ganglia or temporal lobar intracerebral hemorrhage (ICH) models and were divided into 6 hours, 12 hours, 24 hours, 48 hours and 72 hours after hemorrhage And 96h group, 5 rats in each group. The levels of TNFα in each group were measured before sacrifice and at the corresponding time before sacrifice. The levels of TNFα in perihematoma were measured after sacrifice and the infiltrating leukocytes and apoptotic neurons were counted and compared with the healthy control group (n = 5) Compare Results The levels of TNFα in perihematoma and perihematoma of model dogs reached their peak at 24h and 48h after hemorrhage, respectively (r = 0.914, P <0.05). The peak of apoptosis and leukocyte infiltration in perihepatic lesions reached peak at 48h (R = 0.971, P <0.05). There was a positive correlation between TNFα and tumor necrosis factor (r = 0.949 and 0.801, all P <0.05). Conclusion TNFα and leukocyte infiltration are involved in the process of brain injury after intracerebral hemorrhage, both of which may be related to neuronal apoptosis.
