目的探讨宫颈小细胞癌(SCCC)与亮氨酸拉链蛋白(FEZ1)的关系及临床价值。方法回顾性分析2009年1月至2014年10月间接受治疗的25例SCCC患者的临床资料。另外选取同期首次确诊的29例宫颈慢性炎患者和29例宫颈上皮内瘤变(CINⅢ)者、29例宫颈其他类型恶性肿瘤患者分别作为对照1组、对照2组和对照3组。比较SCCC组与对照3组患者的一般临床资料、FEZ1蛋白的表达情况以及临床病理特征与FEZ1蛋白的关系。结果 SCCC组与对照3组患者的平均总生存期(OS)、平均无进展生存期(PFS)、淋巴结转移率以及化疗比率间差异有统计学意义(P<0.01),国际妇产联盟分期(FIGO分期)、肿瘤直径等差异无统计学意义(P>0.05)。对照1组患者的FEZ1阳性表达率最高,其次为对照2组和对照3组,而SCCC组患者的FEZ1阳性表达率最低。SCCC组与各对照组的FEZ1蛋白阳性表达率间差异有统计学意义(均P<0.05)。有淋巴结转移者与无淋巴结转移者以及低、中、高分化者间的FEZ1蛋白阳性表达率差异均有统计学意义(均P<0.05)。结论 FEZ1蛋白阳性表达的缺失与SCCC的发生以及淋巴结转移相关,是辅助诊断SCCC的一个重要的潜在免疫组化指标。
Objective To investigate the relationship between cervical small cell carcinoma (SCCC) and leucine zipper protein (FEZ1) and its clinical value. Methods The clinical data of 25 SCCC patients who were treated between January 2009 and October 2014 were retrospectively analyzed. In addition, 29 cases of chronic cervicitis and 29 cases of cervical intraepithelial neoplasia (CINⅢ) were selected for the first time in the same period, and 29 cases of other types of cervical cancer were selected as control group 1, control group 2 and control group 3 respectively. The clinical data, the expression of FEZ1 protein and the clinicopathological features of FEZ1 protein in SCCC group and control group were compared. Results There were significant differences in mean overall survival (OS), mean progression-free survival (PFS), lymph node metastasis and chemotherapy between SCCC group and control group (P <0.01) FIGO staging), tumor diameter and other differences were not statistically significant (P> 0.05). The control group 1 had the highest FEZ1 positive rate, followed by the control group 2 and the control group 3, while the SCZ group had the lowest FEZ1 positive rate. The positive rate of FEZ1 protein in SCCC group and each control group was significantly different (all P <0.05). The positive expression rate of FEZ1 protein between patients with lymph node metastasis and without lymph node metastasis, as well as between low, moderate and high differentiated patients was significantly different (all P <0.05). Conclusion The lack of positive expression of FEZ1 protein is associated with the occurrence of SCCC and lymph node metastasis, which is an important potential immunohistochemical indicator for diagnosis of SCCC.
