目的探讨中性粒细胞碱性磷酸酶(NAP)染色在部分白血病治疗中的应用价值。方法采用偶氮偶取法(Kaplow法),选取120例已经确诊的四类白血病,其中急性淋巴细胞白血病(ALL)41例,急性髓性白血病(AML)49例,慢性淋巴细胞白血病(CLL)10例,慢性粒细胞白血病(CML)20例,分别在治疗前和治疗后(骨髓象达到部分或完全缓解的标准)采集手指末梢血涂片做中性粒细胞碱性磷酸酶染色,并分别统计四组白血病在治疗前后中性粒细胞碱性磷酸酶染色的阳性积分值。结果 ALL组和CLL组在治疗前NAP染色积分较高,治疗后NAP染色积分明显下降;AML组和CML组治疗前NAP染色积分较低,治疗后NAP染色积分明显上升,四组白血病患者治疗前后NAP染色积分均有较明显的变化,差异有统计学意义。结论中性粒细胞碱性磷酸酶染色除了可以帮助诊断鉴别部分白血病,还可以作为判断部分白血病疗效的指标之一。
Objective To investigate the value of neutrophil alkaline phosphatase (NAP) staining in the treatment of partial leukemia. Methods A total of 120 cases of four types of leukemia confirmed by Kapuz method were selected. Among them, 41 cases were acute lymphoblastic leukemia (ALL), 49 cases were acute myeloid leukemia (AML), 10 cases were chronic lymphocytic leukemia (CLL) Cases, chronic myeloid leukemia (CML) 20 cases, respectively, before and after treatment (bone marrow to achieve partial or complete remission of the standard) to collect finger peripheral blood smear for neutrophil alkaline phosphatase staining and statistics Four groups of leukemia before and after treatment of neutrophil alkaline phosphatase staining positive integrated value. Results The NAP staining scores of ALL group and CLL group were higher before treatment, while the scores of NAP staining decreased significantly after treatment. The score of NAP staining in AML group and CML group was lower before treatment, and the score of NAP staining increased significantly after treatment. NAP staining scores have more significant changes, the difference was statistically significant. Conclusion Neutrophil alkaline phosphatase staining in addition to help identify some of the diagnosis of leukemia, but also can be used as one of the indicators to determine the efficacy of some leukemia.
