为探讨高频喷射通气(HFJV)治疗慢性肺心病并Ⅱ型呼衰的时机,笔者观察了20例慢性肺心病并Ⅱ型呼衰患者,并随机分两组。Ⅰ组神志清楚,Ⅱ组神志不清,有意识障碍,通气4小时。结果表明:两组通气后PaO_2均明显升高(P<0.01),Ⅰ组通气后SaO_2明显升高(P<0.01)。Ⅱ组SaO_2虽也明显升高(P<0.05),但较Ⅰ组略低。Ⅰ组通气后PaCO_2变化不大(P>0.05)Ⅱ组通气后PaCO_2明显升高(P<0.01),Ⅰ组通气后PH无明显变化(P>0.05), Ⅱ组通气后pH值明显降低(P<0.05).
To investigate the timing of high frequency jet ventilation (HFJV) treatment of chronic pulmonary heart disease and type Ⅱ respiratory failure, the author observed 20 patients with chronic pulmonary heart disease and type Ⅱ respiratory failure, and were randomly divided into two groups. Group Ⅰ conscious clear, Ⅱ group unconsciousness, disturbance of consciousness, ventilation for 4 hours. The results showed that the PaO_2 in both groups increased significantly after ventilation (P <0.01), and SaO_2 in group Ⅰ increased significantly after ventilation (P <0.01). Although SaO 2 in group Ⅱ was significantly increased (P <0.05), it was slightly lower than that in group Ⅰ. PaCO_2 in group Ⅰ was not changed much after ventilation (P> 0.05), PaCO_2 in group Ⅱ was significantly increased after ventilation (P <0.01), but PH in group Ⅰ was not changed significantly after ventilation (P> 0.05) P <0.05).
