目的研究探讨分析X线,CT,低场MRI在诊断糖尿病足时的影像学表现,分析它们的诊断价值。方法2007年9月至2015年5月收治的37例有糖尿病足的患者,首先用X线DR平片及双能X线骨密度的仪器对于全部患者进行检查,将其中32例糖尿病足患者再行螺旋CT检查,5例未见X线异常做低场MRI检查,对于这三种检查方式进行比较分析。结果糖尿病足的X线表现:骨质疏松,骨端骨质破坏,神经性骨关节病,软组织肿胀,血管壁钙化;CT表现:与X线相似,下肢远端血管病变得到很好地显示,特别是血管壁钙化优于X片;MRI表现:软组织水肿,溃疡,骨髓炎,关节腔积液,关节脱位。结论 X线平片对于糖尿病足晚期的骨质及血管改变显示良好,而CT能够显示细微骨质变化,且优于X片,还可对于血管壁钙化清楚显示,MRI对于早期骨髓炎显示优于X线平片,还能够确定感染部位及范围。
Objective To study and analyze the imaging findings of X-ray, CT and low-field MRI in the diagnosis of diabetic foot and to analyze their diagnostic value. Methods From March 2007 to May 2015, 37 patients with diabetic foot were examined by X-ray plain radiography and dual-energy X-ray absorptiometry. Results X-ray manifestations of diabetic foot: osteoporosis, osteoinductive bone destruction, neurogenic osteoarthritis, soft tissue swelling, calcification of the vessel wall; CT performance: similar to the X-ray, distal lower extremity vascular disease has been well demonstrated, Especially vascular wall calcification is superior to X-ray; MRI manifestations: soft tissue edema, ulcers, osteomyelitis, joint effusion, joint dislocation. Conclusion X-ray film showed good bone and vascular changes in the late stage of diabetic foot, and CT can show fine bone changes, and better than the X-ray, but also for the calcification of the blood vessel clearly shows that MRI showed better for early osteomyelitis X-ray film, but also to determine the location and extent of infection.
