[目的]应用经尿道前列腺剜除术(TERP)治疗前列腺增生,并同时与应用经尿道前列腺电切术(Transurethral resection of the prostate,TURP)治疗前列腺增的疗效作比较。[方法]64例BPH患者根据入院先后顺序随机分为观察组(经尿道前列腺剜除术,TERP)和对照组(经尿道前列腺电切术,TURP)两组,每组32例,观察比较两组的手术时间、术中出血量、腺体切除量、导尿管留置时间、术后住院时间;并发症:低血钠、尿失禁、继发出血及尿道狭窄;国际前列腺症状IPSS、生活质量评分QOL、最大尿流率Qmax、膀胱残余尿PUV。[结果]观察组患者手术所用时间、术中出血量少、腺体切除量、导尿管留置时间、术后住院时间均明显少于对照组(P﹤0.05)。对照组发生低血钠的发生率(37.5%)明显高于观察组(9.34%)(P﹤0.05)。两组术后IPSS、QOL、Qmax、PUV均较术前IPSS、QOL、Qmax、PUV明显改善(P﹤0.05),但两组间比较差异无统计学意义。[结论]与应用经尿道前列腺电切术比较,经尿道前列腺剜除术治疗BPH具有手术时间短,术中出血少,术后并发症少等优点,值得广泛推广和应用。
[Objective] To compare the efficacy of transurethral resection of the prostate (TURP) for the treatment of benign prostatic hyperplasia (BPH) treated with transurethral resection of the prostate (TERP). [Methods] 64 patients with BPH were randomly divided into observation group (transurethral epigastric surgery, TERP) and control group (transurethral resection of prostate, TURP), with 32 cases in each group. Group surgery time, intraoperative blood loss, gland resection, catheter retention time, postoperative hospital stay; complications: hyponatremia, urinary incontinence, secondary bleeding and urethral stricture; International Prostate Symptom IPSS, quality of life Scoring QOL, maximal flow rate Qmax, bladder residual urine PUV. [Results] The operation time, intraoperative blood loss, gland removal, catheterization time and postoperative hospital stay in the observation group were significantly less than those in the control group (P <0.05). The incidence of hyponatremia in the control group (37.5%) was significantly higher than that in the observation group (9.34%) (P <0.05). The IPSS, QOL, Qmax and PUV of the two groups were significantly improved (P <0.05) compared with the preoperative IPSS, QOL, Qmax and PUV, but there was no significant difference between the two groups. [Conclusion] Compared with transurethral resection of prostate, transurethral resection of BPH has the advantages of short operation time, less intraoperative bleeding and less postoperative complications, which deserves wide application.
