为了探讨肾上极筋膜悬吊法与后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿的疗效,分析我院收治89例行后腹腔镜肾蒂淋巴管结扎术患者的临床资料.按手术方法分为肾上极筋膜悬吊结扎组(43例)和传统手术组(46例).对比两组患者临床数据(年龄、性别、左右侧、BMI)、围手术期数据(手术时间、术中出血量、术后胃肠通气时间、术后住院日、术后3个月血红蛋白和血清白蛋白值).结果显示肾上极筋膜悬吊结扎法手术组手术时间明显缩短,两者比较差异有统计学意义(t=12.3,P＜0.05);比较2组数据术中出血量、术后胃肠通气时间、术后住院天数、术后3个月血红蛋白及血清白蛋白变化值均无统计学意义(P＞0.05).因此肾上极筋膜悬吊结扎法后腹腔镜肾蒂淋巴管剥脱术减少手术步骤,降低手术难度,缩短手术时间.“,”To compare efficacy of retroperitoneal approach laparoscopic renal pedicle lymphatic disconnection via suspension and ligation of the fascia of upper pole of kidney and traditional laparoscopic for chyluria.Clinical data on 89 patients who underwent retroperitoneal approach laparoscopic renal pedicle lymphatic disconnection for chyluria in our hospital were retrospectively analyzed.The patients were divided into 2 groups:observation group (n =43,received retrbperitoneal approach laparoscopic renal pedicle lymphatic disconnection via suspension and ligation of the fascia of upper pole of kidney) and control group (n=46,received traditional retroperitoneal approach laparoscopic technology).The demographic data (age,gender,laterality,BMI),perioperative indexes and clinical outcomes (operation time,intraoperative blood loss,postoperative gastrointestinal recovery time,postoperative hospital stay,three months after the operation of hemoglobin and serum albumin) were compared between the 2 groups.The results showed that the operation time in the observation group was shorter than that in the control group,and the difference was statistically significant (t =12.3,P＜0.05).There were no differences in intraoperative blood loss,postoperative gastrointestinal recovery time,postoperative hospital stay and three months after the operation of hemoglobin and serum albumin between the 2 groups (P ＞ 0.05).Retroperitoneal approach laparoscopic renal pedicle lymphatic disconnection via suspension and ligation of the fascia of upper pole of kidney has the advantages of shorter operation time.
