[目的]了解贵德县新生儿乙型肝炎疫苗接种效果,为指导和落实乙肝免疫预防策略提供科学依据。[方法]对2003～2010年贵德县乙肝疫苗接种资料、2008～2010年每年对托幼机构全程接种乙肝疫苗后的1～6岁儿童开展乙肝病毒表面抗原(HBsAg)、乙肝病毒表面抗体(抗-HBs)检测资料进行分析。[结果]2003～2010年合计接种新生儿10 720人,应接种10 955人,全程接种率为97.85%;首剂及时接种9 247人,应接种11 088人,首剂及时接种率为83.40%。全程、首针及时报告接种率,2003年分别为90.23%、40.30%,2010年分别为99.67%、98.73%。2008～2010年合计调查全程接种乙肝疫苗后的儿童1 446名,抗-HBs阳性的1 125人,阳性率为77.80%。抗-HBs阳性率,各年份分别为71.60%、78.87%、81.75%(P<0.01);城镇、农村、牧区分别为81.87%、78.09%、65.50%(P<0.01);≤2岁为82.05%、3岁为83.51%、4岁为77.72%,5岁为76.19%,6岁为69.17%(P<0.01)。[结论]贵德县乙肝疫苗全程接种率和首剂及时率逐年提高,但抗-HBs阳性率随年龄增长呈下降趋势。
[Objective] To understand the effect of hepatitis B vaccination in Newborns in Guide County and provide scientific basis for guiding and implementing hepatitis B immune prevention strategy. [Method] According to the hepatitis B vaccination data in Guide County from 2003 to 2010, hepatitis B virus surface antigen (HBsAg), hepatitis B virus surface antibody (HBsAg) and hepatitis B virus (HBsAg) were detected in children aged 1 ~ 6 years after inoculation of hepatitis B vaccine Anti-HBs) test data for analysis. [Results] A total of 10 720 newborns were vaccinated in 2003-2010, and 10 955 should be inoculated with a total inoculation rate of 97.85%. 9 247 persons were vaccinated in time for the first dose and 11 088 persons were inoculated. The timely inoculation rate of the first dose was 83.40% . In the whole process, the first-to-be-reported promptly reported inoculation rates were 90.23% and 40.3% respectively in 2003 and 99.67% and 98.73% respectively in 2010. The total number of children vaccinated with hepatitis B vaccine during the entire survey from 2008 to 2010 was 1 446 and 1 125 anti-HBs positive, with a positive rate of 77.80%. The positive rate of anti-HBs was 71.60%, 78.87%, 81.75% (P <0.01) in each year, and 81.87%, 78.09% and 65.50% in urban areas, rural areas and pastoral areas respectively (P <0.01) %, 83.51% at 3 years, 77.72% at 4 years, 76.19% at 5 years and 69.17% at 6 years (P <0.01). [Conclusion] The whole vaccination rate and first dose and timely rate of Hepatitis B vaccine in Guide County increased year by year, but the positive rate of anti-HBs showed a decreasing trend with age.
