应用聚合酶链反应(PCR)技术对45例胸水、3例脑脊液(CSF)进行结核菌DNA检测,并同时与胸水及CSF涂片抗酸染色进行比较。结果:30例结核性胸水涂片抗酸染色和PCR检测阳性率分别为0%和43.3%,后者显著高于前者(P<0.01)。结核性脑膜炎CSF的PCR检测阳性率为50%。15例非结核性胸水PCR阳性率为6.7%,涂片均阴性。说明PCR检测胸水及脑脊液结核菌DNA具有较高敏感性和特异性。对检测过程中出现的假阳性和假阴性进行分析。
Mycobacterium tuberculosis DNA was detected in 45 cases of pleural effusion and 3 cases of cerebrospinal fluid (CSF) by polymerase chain reaction (PCR), and compared with the acid-fast staining of pleural effusion and CSF smear. Results: The positive rates of acid-fast staining and PCR in 30 cases of tuberculous pleural effusion were 0% and 43.3% respectively, the latter was significantly higher than the former (P <0.01). PCR detection of tuberculous meningitis CSF positive rate of 50%. 15 cases of non-tuberculous pleural effusion PCR positive rate was 6.7%, smears were negative. PCR detection of pleural fluid and cerebrospinal fluid of M. tuberculosis DNA has a higher sensitivity and specificity. The false positives and false negatives that occurred during the test were analyzed.
