目的探讨磁共振成像在骨结核和骨肿瘤中的临床诊断价值。方法选取2016年1月-2017年1月本院经病理检查证实的骨结核患者21例和骨肿瘤患者18例,均进行磁共振成像(MRI)检查,对其影像学检查结果进行分析。结果经MRI检查,骨结核患者影像学表现为骨质疏松,骨破坏,骨质增生,死骨钙化,关节及周围软组织肿胀;骨肿瘤患者影像学表现为侵犯软组织;其中骨肿瘤诊断准确率为94.44%,骨结核诊断准确率为90.48%,差异无统计学意义(P>0.05);当b=150s/mm~2时,骨结核和骨肿瘤患者ADC值差异不明显(P>0.05);当b=300s/mm~2时,骨结核ADC值比骨肿瘤高(P<0.05)。结论采取MRI检查的诊断准确率均较高,能有效鉴别骨结核和骨肿瘤,具有良好的临床诊断价值。
Objective To investigate the clinical value of magnetic resonance imaging in bone tuberculosis and bone tumors. Methods Twenty-one cases of bone tuberculosis and 18 cases of bone tumor confirmed by pathology in our hospital from January 2016 to January 2017 were examined by magnetic resonance imaging (MRI) and their imaging findings were analyzed. Results MRI examination showed osteoporosis, bone destruction, hyperosteogeny, sequestrum calcification, swelling of joints and surrounding soft tissue in imaging diagnosis of bone tuberculosis. Imaging features of bone cancer were infiltrating soft tissue. The diagnostic accuracy of bone tumors was 94.44%, and the diagnostic accuracy of bone tuberculosis was 90.48% (P> 0.05). When b = 150s / mm ~ 2, there was no significant difference in ADC value between bone tuberculosis and bone tumor (P> 0.05). When b = 300s / mm ~ 2, the ADC value of bone tuberculosis was higher than that of bone tumors (P <0.05). Conclusion The diagnostic accuracy of MRI examination is high, which can effectively distinguish the bone tuberculosis and bone tumors, and has a good clinical diagnostic value.
