作者介绍用连续聚丙烯酰胺梯度凝胶微电泳(PAGE)区分尿蛋白型以鉴别血尿来源。经分析62例血尿标本的PAGE检查结果,发现高分子型(含巨球蛋白)多出现于非肾小球性血尿,而肾小球性血尿则基本无巨球蛋白发现。以巨球蛋白作为鉴别血尿的标准,其敏感性和特异性均达到95%。这是一种客观性强,诊断符合率高,重复性好的无创伤性血尿筛选方法,但对不含尿蛋白的单纯血尿则鉴别诊断价值较小。
The authors describe the use of continuous polyacrylamide gradient gel electrophoresis (PAGE) to distinguish urine protein type to identify the source of hematuria. After analysis of PAGE results of 62 cases of hematuria, it was found that macromolecules (including macroglobulin) appeared in non-glomerular hematuria, while glomerular hematuria was found to be almost without macroglobulin. With macroglobulin as the standard to identify hematuria, the sensitivity and specificity are up to 95%. This is a noninvasive screening method of hematuria with strong objectivity, high coincidence rate and good repeatability. However, the differential diagnosis of simple hematuria without urinary protein is of less value.
