目的检测类风湿关节炎(RA)患者血浆D-二聚体(DD)及抗凝血酶Ⅲ(AT-Ⅲ)表达水平,探讨其在RA中的临床意义。方法选择RA患者36例为RA组,正常对照组30例,测定其血浆DD、AT-III及血浆C反应蛋白(CRP)水平。结果 RA组患者血浆DD水平为(289.36±71.75)μg/L,显著高于正常对照组的(132.25±38.63)μg/L;血浆AT-III水平为(56.2±12.70)%,明显低于对照组的(92.40±3.80)%,差异均有统计学意义(P<0.05)。相关性分析显示,RA组患者血浆DD、AT-III水平与DAS28评分具有相关性(r=0.32,P<0.01;r=-0.72,P<0.01)。RA组患者经治疗病情缓解后(DAS28<2.6)血浆DD水平为(141.38±32.75)μg/L,显著低于治疗前的(289.36±71.75)μg/L,差异有统计学意义(P<0.01),与正常对照组比较差异无统计学意义(P>0.05);RA组患者经治疗病情缓解后血浆AT-III水平为(96.20±4.20)%,显著高于治疗前的(56.2±12.7)%,差异有统计学意义(P<0.01),与正常对照组比较差异无统计学意义(P>0.05)。结论血浆DD及AT-III水平与RA患者的病情活动有关,可反映心血管病变危险性,对RA患者的病情判断及预后估计有重要的评判价值。
Objective To detect the expression of plasma D-dimer (DD) and antithrombin Ⅲ (AT-Ⅲ) in patients with rheumatoid arthritis (RA), and to explore its clinical significance in RA. Methods Thirty-six RA patients were selected as RA group and 30 normal control subjects. Plasma DD, AT-III and plasma C-reactive protein (CRP) levels were measured. Results The level of plasma DD in patients with RA was (289.36 ± 71.75) μg / L, which was significantly higher than that of the control group (132.25 ± 38.63) μg / L and that of plasma AT-III was 56.2 ± 12.70% Group (92.40 ± 3.80)%, the difference was statistically significant (P <0.05). Correlation analysis showed that plasma DD, AT-III levels in RA patients were correlated with DAS28 scores (r = 0.32, P <0.01; r = -0.72, P <0.01). The plasma AT-III level in the RA group was (96.20 ± 4.20)% after treatment and was significantly higher than that before treatment (56.2 ± 12.7) %, The difference was statistically significant (P <0.01), compared with the normal control group, the difference was not statistically significant (P> 0.05). Conclusions The plasma levels of DD and AT-III are correlated with the severity of RA in patients with RA. They may reflect the risk of cardiovascular disease and have important value in judging the prognosis and prognosis of patients with RA.
