目的探讨脓毒症患儿血清炎症因子及血淀粉酶、脂肪酶水平变化,并研究其临床意义。方法选取脓毒症患儿66例为研究对象,根据是否继发胰腺功能损伤将受试患儿分为单纯脓毒症组33例和胰腺功能损伤组33例,同时选取33例健康儿童为健康组。对三组研究对象血清白介素-1β(IL-1β)、白介素-6(IL-6)、白介素-8(IL-8)、C反应蛋白(CRP)及血淀粉酶、脂肪酶水平进行检测,并分析其相关性。结果单纯脓毒症组儿童血清IL-1β、IL-6、IL-8及CRP水平均显著高于健康组(P<0.05),而血淀粉酶和脂肪酶水平与健康组差异无显著性(P>0.05)。胰腺功能损伤组儿童血清IL-1β、IL-6、IL-8、CRP水平及血淀粉酶、脂肪酶水平均显著高于单纯脓毒症组和健康组(P<0.05)。相关性分析表明,胰腺功能损伤组和单纯脓毒症组儿童血清IL-1β、IL-6、IL-8、CRP与血淀粉酶、脂肪酶水平均呈显著正相关(P<0.05)。结论脓毒症继发胰腺功能损伤儿童血中炎症因子和淀粉酶、脂肪酶水平均显著升高,且存在显著相关性。
Objective To investigate the changes of serum inflammatory factors and serum amylase and lipase in children with sepsis and to study its clinical significance. Methods Sixty-six children with sepsis were enrolled in this study. According to whether secondary pancreatic injury occurred or not, thirty-three children with sepsis and thirty-three patients with impaired pancreatic function were divided into three groups: healthy children group. The levels of serum IL-1β, IL-6, IL-8, C-reactive protein (CRP) and serum amylase and lipase in the three groups were detected, And analyze its relevance. Results Serum levels of IL-1β, IL-6, IL-8 and CRP in sepsis group were significantly higher than those in healthy group (P <0.05), but there was no significant difference between serum amylase and lipase levels P> 0.05). Serum levels of IL-1β, IL-6, IL-8 and CRP, serum amylase and lipase in children with pancreatic impairment group were significantly higher than those in simple sepsis group and healthy group (P <0.05). Correlation analysis showed that the levels of IL-1β, IL-6, IL-8 and CRP were positively correlated with serum amylase and lipase in pancreas injury group and simple sepsis group (P <0.05). Conclusion The levels of inflammatory cytokines and amylase and lipase in blood of children with sepsis secondary to pancreatic function damage were significantly increased, and there was a significant correlation between them.
