目的 :应用经颅磁刺激运动诱发电位技术 (TMS -MEP) ,检测运动神经元病 (MND)中枢运动通路传导功能 ,以明确此技术在运动神经元病中的诊断价值。方法 :MND患者 2 0例 ,其中肌萎缩侧索硬化 (ALS) 13例 ,脊髓肌萎缩症(SMA) 7例 ;健康对照组 2 0例。以健康受试者为标准 ,测定MND患者上肢运动诱发电位 (MEP) ,以中枢运动传导时间(CMCT)延长及波形消失为异常判定标准。结果 :MND患者中 ,CMCT延长和波形消失分别为 3 0 .0 %和 3 5 .0 % ,总异常率达 65 .0 %。TMS MEP在ALS和SMA中的异常率分别为 95 .5 %和 14 .3 % (P <0 .0 1)。结论 :TMS MEP在MND的诊断和分型中具有重要的临床应用价值
Objective: To evaluate the diagnostic value of this technique in motor neuron disease by using transcranial magnetic stimulation motor evoked potentials (TMS-MEP) to detect the conduction function of central motor pathway in motor neuron disease (MND). Methods: Twenty patients with MND were enrolled, including 13 patients with amyotrophic lateral sclerosis (ALS), 7 patients with spinal muscular atrophy (SMA) and 20 healthy controls. Taking healthy subjects as the standard, the MEP in MND patients was measured. The extension of CMCT and disappearance of the waveform were regarded as abnormal judgment criteria. Results: In patients with MND, the prolongation of CMCT and disappearance of waveforms were 30.0% and 35.0% respectively, with a total abnormal rate of 65.0%. The abnormal rates of TMS MEP in ALS and SMA were 95.5% and 14.3%, respectively (P <0.01). Conclusion: TMS MEP has important clinical value in the diagnosis and classification of MND
