目的 对 1999年某地出现的一批先有腹泻病症状后继发急性肾功能衰竭 (肾衰 )的患者进行血清学调查和诊断。方法 使用基因克隆表达纯化的肠出血性大肠埃希菌 (EHEC) -溶血素(Hly)和EHECO15 7脂多糖 (LPS)为抗原 ,对采集自 4 2例肾衰患者的血清标本进行蛋白印记试验。结果 EHEC Hly的检测发现 ,IgG抗体阳性 2 1份 ,阳性率5 0 .0 % ;IgM抗体阳性 16份 ,阳性率38.1% ;在 11份标本中同时检测到IgG和IgM抗体 ,阳性率2 6 .2 % ;检测到IgG或IgM抗体阳性的标本 2 6份 ,阳性率 6 1.9%。EHECO15 7LPS的检测发现 ,2 4份标本分别检测到IgG和IgM抗体 ,阳性率各为 5 7.1% ;在 2 9份标本中检测到IgG或IgM抗体 ,阳性率6 9.0 % ;在 2 0份标本中检测到IgG和IgM抗体 ,阳性率4 7.6 %。在 4 2份血清标本中同时检测到EHEC Hly和O15 7LPS的特异性抗体的标本 2 2份 ,阳性率 5 2 .4 % ;检测到EHEC Hly或O15 7LPS的特异性抗体的标本 34份 ,阳性率81.0 %。结论 结合细菌学和血清学诊断结果、临床症状和流行病学分析 ,可以认为这些患者感染了EHECO15 7∶H7。在分离不到病原菌的情况下 ,检测患者血清标本的EHEC Hly和 /或EHECO15 7LPS的特异性抗体 ,不失为一种可以考虑的诊断方法
Objective To carry out serological investigation and diagnosis of a group of patients with acute renal failure (renal failure) after the first symptoms of diarrhea in 1999. Methods Serum samples collected from 42 patients with renal failure were tested by Western blot using genetically modified EHEC-hemolysin and EHECO157 lipopolysaccharide (LPS) as antigen. . IgM antibodies were positive in 16 samples and the positive rate was 38.1%. IgG and IgM antibodies were detected in 11 samples with a positive rate of 26 .2% of the samples tested positive for IgG or IgM antibody, the positive rate was 6 1.9%. EHECO15 7LPS test found 24 cases were detected IgG and IgM antibodies, the positive rate of 51.1% for each; in 29 samples were detected IgG or IgM antibodies, the positive rate of 6 9.0%; in 20 specimens In the detection of IgG and IgM antibodies, the positive rate of 4 7.6%. In 42 serum samples, two samples of EHEC Hly and O15 7LPS specific antibodies were detected at the same time, with a positive rate of 5.2.2%. 34 samples of EHEC Hly or O15 7LPS specific antibodies were detected, Rate of 81.0%.
