目的应用扩散张量成像技术(diffusion tensor imaging,DTI)研究平均扩散系数(average diffusion coeffi-cient,DCavg)在缺血性脑梗死不同临床时期的变化特点。方法对79例不同临床时期的缺血性脑梗死患者行常规磁共振成像及DTI检查。分别测量不同临床时期患者梗死区与健侧的DCavg值,并计算其相对值(rDCavg);比较同一临床时期梗死区与健侧的DCavg值,分析不同临床时期间rDCavg值的变化趋势。结果超急性期、急性期、亚急性期患者DCavg值为不同程度地降低;大部分慢性早期及慢性晚期患者DCavg值为不同程度地升高。结论缺血性脑梗死不同临床时期DCavg值的变化有一定规律,据此可揭示脑梗死病变的病理生理演变过程、判断所处临床时期。
Objective To investigate the changes of average diffusion coefficient (DCavg) in different clinical stages of ischemic stroke by diffusion tensor imaging (DTI). Methods 79 patients with different clinical stages of ischemic stroke patients underwent routine magnetic resonance imaging and DTI examination. The DCavg values of infarct area and contralateral side in different clinical periods were respectively measured and their relative values (rDCavg) were calculated. The DCavg values of infarct area and contralateral side in the same clinical period were compared to analyze the changing trend of rDCavg values in different clinical periods. Results The values of DCavg in patients with hyperacute, acute and subacute phases decreased to different extents; the DCavg values of most patients with chronic early and chronic phase increased to varying degrees. Conclusion The change of DCavg value in different clinical stages of ischemic cerebral infarction has some rules, which can reveal the pathophysiological evolution process of cerebral infarction and determine the clinical stage.
