目的探讨骨质疏松分级与老年粗隆间骨折内固定的关系。方法对150例接受内固定手术的老年股骨粗隆间骨折的患者进行回顾性研究,通过总结患者术后内固定的失败率,分析骨质疏松分级和内固定失败的关系,并进行统计学分析。结果本组术后内固定失败23例,包括:钢板断裂松动,螺钉拔出松动,骨皮质劈裂,股骨干劈裂骨折等,失败率为15.3%。Singh分级越高,失败率越高。结论老年人股骨粗隆间骨折内固定的选择应与Singh骨质疏松分级相结合,在确保手术创伤小,功能恢复快的前提下选择合适、有效的内固定,必要时植骨,填充骨水泥,可降低内固定失败率。
Objective To investigate the relationship between osteoporosis classification and internal fixation of elderly intertrochanteric fractures. Methods A retrospective study was conducted on 150 elderly patients with intertrochanteric fractures treated with internal fixation. The failure rate of internal fixation after operation was analyzed to analyze the relationship between the classification of osteoporosis and the failure of internal fixation. The statistical analysis was performed . Results Twenty-three cases failed internal fixation after this procedure, including loose loosening of the plate, loosening of the screw, cortical cleavage and splitting of the femoral shaft. The failure rate was 15.3%. The higher the Singh rating, the higher the failure rate. Conclusion The selection of internal fixation of intertrochanteric fractures in the elderly should be combined with Singh osteoporosis grading. Choosing suitable and effective internal fixation under the premise of ensuring surgical trauma and quick recovery of function, if necessary, bone grafting and filling of bone cement , Can reduce the failure rate of internal fixation.
