目的:观察加味温胃汤联合三联疗法治疗幽门螺旋杆菌(Hp)感染致胃脘痛的疗效及对胃液成分、Hp根除率的影响。方法:选取我院2014年5月~2015年5月92例胃脘痛患者为研究对象,将患者抽签随机分为观察组与对照组,每组46例。对照组给予三联疗法治疗,观察组在对照组的基础上另给予加味温胃汤治疗,比较两组患者临床疗效,氨基己糖、三羟胆酸、胃液分泌性免疫球蛋白、胃蛋白酶等胃液成分,Hp根除率、复发率及不良反应发生率。结果:观察组有效率93.48%高于对照组78.26%,有统计学意义(P<0.05)。两组治疗后氨基己糖、三羟胆酸均低于治疗前,胃液分泌性免疫球蛋白、胃蛋白酶高于治疗前,有统计学意义(P<0.05);观察组氨基己糖(112.34±20.36)μg/ml、三羟胆酸(21.36±7.59)μg/ml低于对照组,胃液分泌性免疫球蛋白(62.35±6.29)μg/ml、胃蛋白酶(37.26±6.29)μg/ml高于对照组,有统计学意义(P<0.05)。观察组Hp根除率91.30%高于对照组73.91%,复发率21.74%低于对照组43.48%,有统计学意义(P<0.05)。两组并发症发生率比较无统计学意义(P>0.05)。结论:加味温胃汤联合三联疗法治疗Hp感染致胃脘痛,可有效提高临床疗效及Hp根除率,保护胃黏膜,调整胃液分泌,降低复发率,且不良反应少,具有较好的临床应用价值。
Objective: To observe the therapeutic effect of Jiaweibweisi Decoction combined with triple therapy on gastric ulcer caused by Helicobacter pylori (Hp) infection and its effect on gastric juice composition and Hp eradication rate. Methods: From May 2014 to May 2015 in our hospital, 92 cases of stomach epigastric pain were selected as the research object. Patients were randomized into observation group and control group with random sampling, 46 cases in each group. The control group was treated with triple therapy. The observation group was given Jiaweifweisi Decoction on the basis of the control group, and the clinical curative effect, aminohexose, trihydroxycholic acid, gastric secreting immunoglobulin, pepsin and other gastric juice were compared Composition, Hp eradication rate, recurrence rate and incidence of adverse reactions. Results: The effective rate of the observation group was 93.48% higher than 78.26% of the control group, with statistical significance (P <0.05). The levels of aminocaproate and trihydroxycholic acid in the two groups were all lower than those before treatment, the secreted immunoglobulin and pepsin in gastric juice were higher than those before treatment (P <0.05) (62.35 ± 6.29) μg / ml and pepsin (37.26 ± 6.29) μg / ml respectively, which were higher than that of the control group (20.36) μg / ml and 21.36 ± 7.59 μg / The control group, with statistical significance (P <0.05). The observation group Hp eradication rate was 91.30% higher than the control group 73.91%, the recurrence rate was 21.74% lower than the control group 43.48%, with statistical significance (P <0.05). There was no significant difference in the complication rates between the two groups (P> 0.05). Conclusions: Jiaweifengwei Decoction combined with triple therapy can cure gastric apoplexy caused by Hp infection, which can effectively improve clinical curative effect and Hp eradication rate, protect gastric mucosa, regulate secretion of gastric juice, reduce recurrence rate and less adverse reactions, and has good clinical application value.
