目的布鲁氏菌病(以下简称布病)5种血清学检测方法对青海省三江源地区6 310份人血清进行检测,通过几种方法的对比制定现场调查中最有效、最实用的血清学检测方案。方法以虎红平板凝集试验(RBPT)对三江源地区样本进行布病血清学抗体检测,对RBPT阳性血清进行其它4种试验的检测,分别为试管凝集试验(SAT)、抗人球蛋白试验(Coomb’s)、补体结合试验(CFT)、酶联免疫吸附试验(ELISA)。结果用RBPT方法作为筛查方法对6310份血清进行现场检测,阳性结果为141份,对这141份RBPT阳性血清进行其它4种试验的检测。SAT与ELISA对比,阳性符合率为90.32%、阴性符合率为62.72%、总符合率为68.79%,差异有统计学意义(χ2=29.88,P﹤0.01),ELISA方法的灵敏度为90.32%、特异度为62.73%、约登指数为0.559。SAT与CFT对比阳性符合率为61.29%、阴性符合率为90.91%、总符合率为84.40%,两种血清学试验方法差异无统计学意义(χ2=0.045,P>0.05)。结论目前还没有能用某一种血清学试验方法检测出不同时期出现的抗布鲁氏菌抗体,为能够减少漏检率,建议多种方法联合使用。
Objective To detect 6 310 human serums from brucellosis (hereinafter referred to as brucellosis) on 6 310 human serums in the Three Rivers of Qinghai Province. The sensitivity of ELISA was 90.32% The degree is 62.73%, the Youden index is 0.559. The positive coincidence rate between SAT and CFT was 61.29%, the negative coincidence rate was 90.91% and the total coincidence rate was 84.40%. There was no significant difference between the two serological test methods (χ2 = 0.045, P> 0.05). Conclusion At present, there is not any anti-Brucella antibody that can be detected in a certain serological test at any time. To reduce the missed detection rate, a combination of multiple methods is suggested.
