郭×,男14岁,自幼发现心脏杂音,平时活动后感心慌闷气,并易患呼吸道感染,无紫绀现象。体格检查:胸骨左缘3～4肋间,闻及GⅢ/6连续性杂音。ST(+),P_2稍亢,心功能Ⅲ级。心电图:窦性心律,不完全右束支传导阻滞。彩超提示:心脏左右室均增大,右室大为主。右冠状动脉扩张,直径约2.5cm,且于右室相通,瘘口直径约1.2cm。心脏三位片提示:肺血增多,肺动脉段稍凸,左右室增大。
Guo ×, male, 14 years old, heart murmur was found since childhood, usually after the activity of palpitation and irritability, and susceptible to respiratory infections, no cyanosis. Physical examination: 3 to 4 intercostal sternum left edge, smell and G Ⅲ / 6 continuous murmur. ST (+), P_2 slightly hyperthyroidism, heart function Ⅲ grade. ECG: sinus rhythm, incomplete right bundle branch block. Choi ultrasound tips: the left and right ventricles are increased, right ventricular mass. Right coronary artery dilatation, diameter of about 2.5cm, and in the right ventricle, fistula diameter of about 1.2cm. Three pieces of heart tips: increased pulmonary blood flow, pulmonary artery segment slightly convex, left ventricular enlargement.
