目的:定量分析PTEN蛋白在肺癌、肺良性病变及正常肺组织中的表达特点及其临床意义。方法:用免疫组化SP法检测肺癌组织、肺良性病变和正常肺组织中PTEN蛋白的表达,用LeicaQ500MC图像分析系统对PTEN蛋白表达的阳性强度进行定量测试。结果:PTEN在正常肺组织和肺良性病变中表达的阳性单位(PU值)呈正态分布,在肺癌中呈正偏态分布,三者间的表达差异具有统计学意义(P=0.000);肺癌组织中PTEN的表达显著低于肺良性病变和正常肺组织;肺的鳞癌、腺癌、大细胞癌和小细胞癌中PTEN蛋白表达的PU值基本相同,差异无统计学意义(P=0.789);肺癌中PTEN的表达与肿瘤的大体类型有关(P=0.023),周围型肺癌PTEN蛋白表达的PU值明显低于中央型肺癌;有淋巴结转移的肺癌组织PTEN的PU值(M=8.68)大于无淋巴结转移的肺癌组织(M=5.47,Z=-0.894,P=0.058);肺癌中PTEN的表达强度与患者年龄、性别、吸烟史、癌组织分化程度及pTNM分期均无关(P>0.05),不同肺良性病变(肺大泡、支气管扩张和肺结核)PTEN的表达差异无统计学意义(P=0.889)。结论:肺癌组织PTEN蛋白的表达明显低于非肺癌组织,且呈正偏态分布;周围型肺癌中PTEN的表达低于中央型肺癌。PTEN蛋白的表达有助于肺癌与正常肺组织及不同大体类型肺癌的区别。
Objective: To quantitatively analyze the expression and clinical significance of PTEN protein in lung cancer, benign lung disease and normal lung tissue. Methods: The expression of PTEN protein in lung cancer tissue, benign pulmonary lesion and normal lung tissue was detected by immunohistochemical SP method. The positive expression of PTEN protein was quantified by Leica Q500MC image analysis system. Results: The positive expression of PTEN in normal lung tissues and benign lung lesions was positive (P <0.05), positive in PU (P> 0.05), positive in PU (P = 0.000) The expression of PTEN protein in lung tissues was significantly lower than that in benign lung tissues and normal lung tissues. The PU values of PTEN protein in lung squamous cell carcinoma, adenocarcinoma, large cell carcinoma and small cell carcinoma were the same, with no significant difference (P = 0.789 ). The expression of PTEN in lung cancer was related to the general type of tumor (P = 0.023). The PU value of PTEN protein in peripheral lung cancer was significantly lower than that in central lung cancer. The PU value of PTEN in lung cancer with lymph node metastasis (M = 8.68) The expression of PTEN in lung cancer had no correlation with the age, sex, smoking history, the degree of cancer differentiation and the stage of pTNM (P> 0.05), but was significantly higher than that in patients without lymph node metastasis (M = 5.47, Z = ). There was no significant difference in the expression of PTEN in different benign lung lesions (bullae, bronchiectasis and tuberculosis) (P = 0.889). CONCLUSION: The expression of PTEN protein in lung cancer tissues is significantly lower than that in non-lung cancer tissues, and it is positively skewed. The expression of PTEN in peripheral lung cancer is lower than that in central lung cancer. PTEN protein expression contributes to the difference between lung cancer and normal lung tissue and different general types of lung cancer.
