目的探讨超声引导下经皮微创气管切开术(PDT)在危重症患者人工气道建立中的应用价值。方法选择行PDT的患者31例(治疗组)及传统气管切开的患者27例(对照组),治疗组手术当日行床旁颈部超声检查,观察甲状腺、无名动脉与气管的位置关系,根据超声检查结果进行穿刺。观察两组切口长度、手术时间、术中出血量、切口愈合时间及围手术期、手术后期并发症情况。结果两组切口长度、手术时间、术中出血量及切口愈合时间等均有统计学差异(P均<0.05),治疗组围手术期及手术后期并发症发生率低于对照组(P<0.05)。结论超声引导下PDT是一种微创、快捷、安全的急救技术,适合于ICU的危重患者,在ICU中有较大的应用价值。
Objective To investigate the value of ultrasound-guided percutaneous minimally invasive tracheostomy (PDT) in the establishment of artificial airway in critically ill patients. Methods Totally 31 patients with PDT (treatment group) and 27 patients with tracheotomy (control group) undergoing trabeculectomy were enrolled in this study. The bedside neck ultrasound was performed in the treatment group on the day of operation to observe the relationship between the thyroid and the unnamed artery and the trachea. Ultrasound results puncture. The incision length, operation time, intraoperative blood loss, incision healing time and perioperative and postoperative complications were observed. Results The length of incision, operation time, blood loss and incision healing time were significantly different between the two groups (all P <0.05). The incidence of perioperative and postoperative complications in the treatment group was lower than that in the control group (P <0.05) ). Conclusion Ultrasound-guided PDT is a minimally invasive, fast and safe first aid technique, which is suitable for critically ill patients in ICU and has great value in ICU.
