目的探讨豫北地区肺癌中高危型人乳头瘤病毒(HPVs)感染情况及分型,为初步明确HPVs感染与肺癌的关系提供依据。方法收集新鲜肺癌标本63例,组织离体后分为两部分处理:一部分用于HE染色,以明确病变性质;另一部分用于提取基因组DNA。采用聚合酶链反应(PCR)方法检测肺癌组织中高危型HPVs的感染情况及分型,并分析相关高危型HPVs感染与肺癌组织学类型的关系。结果经病理检验证实,收集的63例肺癌标本包括非小细胞肺癌40例,小细胞肺癌23例。在63例肺癌中,HPV16感染的阳性率为68.25%(43/63),其中65.12%(28/43)为HPV16单独感染,34.88%(15/43)为HPV16与HPV59混合感染。HPV59感染的阳性率为23.81%(15/63),均为与HPV16混合感染。未检测到HPV18、31、33、58和52感染的病例。在非小细胞肺癌和小细胞肺癌中,HPV16阳性率差异有统计学意义(χ2=6.977,P<0.01),而HPV59阳性率差异无统计学意义(χ2=0.086,P>0.05)。结论高危型HPVs,特别是HPV16感染与肺癌存在密切的关系,且与其他类型HPVs的混合感染值得重视。
Objective To investigate the status and classification of high-risk human papillomavirus (HPVs) infection in lung cancer in northern Henan Province, and to provide a basis for initially clarifying the relationship between HPVs infection and lung cancer. Methods Sixty-three fresh lung cancer specimens were collected. The tissues were divided into two parts after treatment: one part was used for HE staining to clarify the nature of the lesions, and the other part was used for the extraction of genomic DNA. Polymerase chain reaction (PCR) was used to detect the infection and classification of high-risk HPVs in lung cancer tissues, and the relationship between high-risk HPVs infection and the histological types of lung cancer was analyzed. Results The pathological examination confirmed that the collected 63 cases of lung cancer included 40 cases of non-small cell lung cancer and 23 cases of small cell lung cancer. In 63 lung cancers, the positive rate of HPV16 infection was 68.25% (43/63), of which 65.12% (28/43) were HPV16 infection alone, and 34.88% (15/43) were HPV16 and HPV59 mixed infection. The positive rate of HPV59 infection was 23.81% (15/63). Both were mixed with HPV16. No cases of HPV 18, 31, 33, 58 and 52 infections were detected. In non-small cell lung cancer and small cell lung cancer, the positive rate of HPV16 was statistically significant (χ2=6.977, P<0.01), while the positive rate of HPV59 was not statistically significant (χ2=0.086, P>0.05). Conclusion There is a close relationship between high-risk HPVs, especially HPV16 infection and lung cancer, and the mixed infection with other types of HPVs deserves attention.
