目的探讨全自动蛋白电泳系统在地中海贫血(简称地贫)筛查中的应用价值,并对5510例测定结果进行分析。方法用法国Sebia公司生产的HydrasysLC电泳仪进行血红蛋白电泳,用其配套的Hyrys扫描系统扫描出各种血红蛋白(Hb)含量,用pH8.6醋酸纤维薄膜定量电泳作HbH、HbBart’s、HbA2含量对比,用醋酸纤维薄膜微量电泳作血红蛋白区带对比;用碱变试验作HbF对比。结果测定标本5510例,醋酸纤维薄膜电泳法及全自动蛋白电泳系统电泳法都检出β地中海贫血480例;α地中海贫血:用pH8.6醋酸纤维薄膜电泳法检出α地中海贫血(HbH病)151例、HbBart’s区带115例,用全自动蛋白电泳系统电泳法检出HbH病148例、HbBart’s区带112例,两种方法检出其它异常Hb都一致:HbCS均为73例、HbE20例、其它异常Hb(HbD、HbG、HbK、HbJ等)区带39例。结论全自动蛋白电泳系统对异常血红蛋白病及β地中海贫血检查有良好的效果;在α地中海贫血的检查中,对HbCS的检出效果非常好,而HbH与HbBart’s区带多较淡染(HbH带尤其明显),偶然出现假阴性结果。在联合分析参数平均红细胞容积(MCV)、平均红细胞血红蛋白含量(MCH)、红细胞脆性试验及必要时行醋酸纤维薄膜微量电泳的基础上,全自动蛋白电泳系统在地中海贫血诊断中有很好的应用价值。
Objective To investigate the value of automatic protein electrophoresis system in the screening of thalassemia (thalassemia), and to analyze the results of 5510 cases. Methods Hydraspheres electrophoresis was performed on a Hydrasys LC manufactured by Sebia Company of France. HbH, HbBart’s and HbA2 contents were scanned by using the Hyrys scanning system. The contents of HbH, HbBart’s and HbA2 were compared with Acetate film microelectrophoresis for hemoglobin zone contrast; Alkali change test for HbF comparison. Results A total of 480 cases of β-thalassemia were detected by electrophoresis of 5510 cases, acetate film electrophoresis and automatic protein electrophoresis system. Α-thalassemia: α-thalassemia was detected by pH8.6 acetate film electrophoresis. 151 cases and 115 cases in HbBart’s area. 148 cases of HbH disease and 112 cases of HbBart’s area were detected by automatic protein electrophoresis system. The other two methods were consistent: HbCS was 73 cases, HbE20 cases, Other abnormal Hb (HbD, HbG, HbK, HbJ, etc.) zone in 39 cases. Conclusion The automated protein electrophoresis system has a good effect on abnormal hemoglobinopathies and β thalassemia. In the detection of α-thalassemia, the detection of HbCS is very good, while HbH and HbBart’s bands are more lightly stained (HbH band Especially obvious), occasionally false negative results. Based on the combined analysis of mean corpuscular volume (MCV), mean erythrocyte hemoglobin (MCH), erythrocyte fragility and acetate acetate film micro-electrophoresis where necessary, the automated protein electrophoresis system is well suited for the diagnosis of thalassemia value.
