目的探讨镍铬合金烤瓷冠的佩戴是否会造成机体的肾功能损伤。方法对795例镍铬合金烤瓷冠佩戴者(接触组)和85例口腔门诊非镍铬合金修复者(对照组)进行烤瓷冠修复的问卷调查和血尿肾功能生化指标的测定,采用两独立样本t检验,单因素方差分析,线性回归模型和logistic回归模型分析佩戴时间、数量和金属裸露水平对肾功能生化指标的影响,P<0.05为差异有统计学意义。结果各血生化指标(血清总蛋白、白蛋白、血肌酐、血尿素氮、血尿酸和肾小球滤过率的估计值)和尿生化指标(尿白蛋白、N-乙酰-β-D-氨基葡糖苷酶、视黄醇结合蛋白和β2-微球蛋白)的实测值和异常率在镍铬合金烤瓷冠的接触组和对照组间差异均无统计学意义(P>0.05)。肾小球滤过率估计值和尿肾功能指标异常率的logistic回归分析显示年龄和体质指数是肾功能异常的两个独立因素,但与镍铬合金烤瓷冠的佩戴时间、佩戴数量和金属内冠裸露水平均不存在相关关系(P>0.05)。结论镍铬合金烤瓷冠的佩戴与机体肾损伤无关。
Objective To investigate whether the wear of nickel-chromium alloy porcelain crowns can cause the renal damage of the body. Methods A total of 795 cases of Ni-Cr alloy porcelain crowns (contact group) and 85 cases of non-Ni-Cr alloy denture patients (control group) were enrolled in the questionnaire survey and the renal biochemical indicators of hematuria. Independent sample t test, one-way ANOVA, linear regression model and logistic regression model were used to analyze the effect of wearing time, quantity and metal exposure on renal biochemical indexes. P <0.05 was considered statistically significant. Results The biochemical parameters (serum total protein, albumin, serum creatinine, blood urea nitrogen, blood uric acid and glomerular filtration rate estimates) and urinalysis (urinary albumin, N-acetyl- Glucosidase, retinol binding protein and β2-microglobulin) measured values and the rate of abnormalities in the Ni-Cr alloy porcelain crowns of the contact group and the control group differences were not statistically significant (P> 0.05). Logistic regression analysis of estimated glomerular filtration rate and abnormal rate of urinary renal function index showed that age and body mass index were two independent factors of renal dysfunction. However, the wearing time of Ni-Cr alloy porcelain crowns, There was no correlation between the bare level of inner crowns (P> 0.05). Conclusion The wear of Ni-Cr alloy porcelain crowns has nothing to do with the renal injury.
