目的:探讨鼻咽组织、外周血白细胞EBV基因组分布及与NPC的关系。方法:采用PCR法检测26例NPC,4例癌旁及14例慢性鼻咽炎鼻咽组织及外周血白细胞中EBVDNA。同时采用酶免疫法测定血清VCA-IgA。结果:1.NPC、癌旁及鼻咽炎组织EBVDNA无明显差异。2.NPC与鼻咽炎组外周血白细胞EBVDNA有明显差异,P<0.05。3.NPC与鼻咽炎组血清VCA-IgA之间存在明显差异。P<0.05。4.同时检测以上三项后三项阳性率及二项阳性率在两组间有显著差异。P<0.05。5.NPC活检组织及血EBVDNA与血清VCA-IgA阳性率之间有良好的一致性。结论:NPC鼻咽组织存在大量的EBVDNA拷贝数,少数分布于外周血白细胞,且与血清VCA-IgA有良好的一致性。3项套式检测比单项鼻咽活检组织EBVDNA或血清VCA-IgA更有助于对NPC的诊断。
Objective: To investigate the distribution of EBV genome in nasopharyngeal and peripheral leukocytes and its relationship with NPC. Methods: EBV DNA was detected by PCR in 26 cases of NPC, 4 cases of paracancerous tissues and 14 cases of chronic nasopharyngitis nasopharyngeal tissues and peripheral white blood cells. At the same time, serum VCA-IgA was measured by enzyme immunoassay. Results: 1. NPC, adjacent and nasopharyngitis EBV DNA no significant difference. 2. EBV DNA of peripheral blood leukocytes in NPC and nasopharyngitis group were significantly different, P <0.05.3. NPC and nasopharyngitis serum VCA-IgA significant difference between. P <0.05.4. At the same time detection of the above three positive rate and positive rate of two significant differences between the two groups. P <0.05.5. There was a good agreement between NPC biopsies and blood EBVNA and serum VCA-IgA positive rates. Conclusion: There are a large number of copies of EBV DNA in nasopharyngeal tissues of NPC, a few are distributed in peripheral leukocytes and have good consistency with serum VCA-IgA. 3 sets of nested detection than nasopharyngeal biopsy EBVDNA or serum VCA-IgA is more helpful for the diagnosis of NPC.
