目的:总结尺骨斜形截骨+局部残存纤维组织成形改良重建环状韧带术治疗儿童陈旧性孟氏骨折的手术方法及治疗效果.方法:收治陈旧性孟氏骨折患儿23例,施行尺骨斜形截骨+局部残存纤维组织成形改良重建环状韧带术,术后定期复查X线片了解骨折愈合情况,检查前臂功能恢复情况,采用Mackay临床功能评定表评估该手术方式的疗效.结果:23例患儿随访1~8年,平均2年,所有病例骨折均愈合良好,根据Mackay评分,优10例,良12例,中1例,优良率为96%.结论:尺骨斜形截骨+局部残存纤维组织成形改良重建环状韧带术治疗儿童陈旧性孟氏骨折,能有效纠正肘关节及前臂畸形,改善功能,手术效果确切,中远期随访疗效满意,是该类损伤的优选治疗方案.
OBJECTIVE: To summarize the operative methods and treatment effects of modified circumferential ligament for the treatment of children’s old Monteggia fracture.Methods: Twenty-three children with old Monteggia fracture were treated with ulnar oblique Shaped osteotomy + local remodeling of fibrous tissue to improve the reconstruction of annular ligament surgery, regular review of X-ray postoperative fracture healing, check the forearm functional recovery, using Mackay clinical function assessment table to assess the efficacy of the surgical results.Results: 23 According to the Mackay score, 10 cases were excellent, 12 cases were good and 1 case was good, the excellent and good rate was 96% .Conclusion: The results of oblique oblique osteotomy + Local residual fibrous tissue forming improved reconstruction of the ligamentous ring ligament for the treatment of children with old Monteggia fractures, can effectively correct the elbow and forearm deformity, improve function, the exact effect of surgery, long-term follow-up effect is satisfactory, is the preferred treatment of such injuries .
