目的探讨胆固醇/高密度脂蛋白-胆固醇比值(TC/HDL-C比值)的高低与脑梗死发病危险性之间的关系,以及这一比值与年龄增长之间的相关性。方法采用回顾性病例对照研究,用全自动血生化分析仪测量各项生化指标,对228例脑梗死患者和229例非脑梗死对照者的血生化指标进行统计分析。结果(1)病例组和对照组的年龄、性别无显著性差异,两组尿素氮(BUN)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、脂蛋白a[Lp(a)]水平无显著性差异(P>0.05);(2)常见脑梗死危险因素如家族史、吸烟史、饮酒史、高血压病史、糖尿病史、病例组明显高于对照组(P<0.05);(3)脑梗死组胆固醇(TC)、甘油三脂(TG)、低密度脂蛋白-胆固醇(LDL-C)水平显著高于对照组(P<0.05),而高密度脂蛋白-胆固醇(HDL-C)水平显著低于对照组(P<0.05),两组血糖水平有显著性差异(P<0.05);(4)病例组的TC/HDL-C比值为(4.26±1.20)mmol/L,对照组为(4.06±1.26)mmol/L。Logistic回归分析显示,调整年龄性别等变量后脑梗死组TC/HDL-C比值显著高于对照组(P<0.05);(5)病例组和对照组合并后按年龄分为四组,对各年龄段组的TC/HDL-C比值进行两两比较,组间没有显著性差异(P>0.05)。结论血浆TC/HDL-C比值≥4.26可能是脑梗死的一个独立危险因素,且这一比值不随年龄的增长而改变。
Objective To explore the relationship between the cholesterol / high density lipoprotein-cholesterol ratio (TC / HDL-C ratio) and the risk of cerebral infarction, and the correlation between this ratio and age. Methods A retrospective case-control study was carried out. The biochemical indexes were measured by automatic blood biochemical analyzer. The blood biochemical indexes of 228 patients with cerebral infarction and 229 non-cerebral infarction patients were statistically analyzed. Results There was no significant difference in the age and sex between the case group and the control group. The levels of BUN, ALT, AST, (P> 0.05); (2) The risk factors of common cerebral infarction such as family history, smoking history, drinking history, history of hypertension, history of diabetes mellitus, the cases were significantly higher than the control group (P <0.05). (3) The levels of TC, TG and LDL-C in cerebral infarction group were significantly higher than those in control group (P <0.05) (P <0.05). (4) The TC / HDL-C ratio in the case group was (4.26 ± 1.20) mmol / L, while the control group was (4.06 ± 1.26) mmol / L. Logistic regression analysis showed that the TC / HDL-C ratio of cerebral infarction group was significantly higher than that of the control group (P <0.05) after adjusting for gender and other variables; (5) The case group and the control group were divided into four groups according to age, The TC / HDL-C ratio in each group was compared with each other without any significant difference (P> 0.05). Conclusion The plasma TC / HDL-C ratio of ≥ 4.26 may be an independent risk factor for cerebral infarction, and this ratio does not change with age.
