目的 通过Pentacam三维眼前节分析系统角膜地形图引导配戴硬性透氧性角膜接触镜(RGPCL),观察RGPCL矫治儿童高度屈光不正和圆锥角膜的临床效果.方法 通过回顾性分析,根据屈光状态及Pentacam三维眼前节分析系统检查的角膜形态,将27例患者(50眼)分为高度屈光不正组和圆锥角膜组.观察两组患儿配戴RGPCL前、配戴后1周、6月和1年时的最佳矫正视力(BCVA),以及圆锥角膜组患儿配戴RGPCL 1年角膜曲率、角膜厚度及角膜前后表面高度的变化.结果 高度屈光不正组和圆锥角膜组配戴RGPCL治疗后1周、6月和1年BCVA与戴镜前BCVA比较,差异均具有统计学意义(P<0.05);高度屈光不正组和圆锥角膜组RGPCL治疗前BCVA差异有统计学意义(P0.05).Pentacam三维眼前节分析系统观察圆锥角膜组角膜相关参数变化,角膜前表面最大曲率值治疗后1年与治疗前比较有所下降,差异有统计学意义(P0.05).结论 配戴RGPCL对高度屈光不正与圆锥角膜儿童是安全有效的,Pentacam眼前节分析系统能够准确检测圆锥角膜患儿的角膜形态变化.“,”Objective To observe the clinical effect of rigid gas permeable contact lens (RGPCL) for the treatment of children with high refractive error and keratoconus by using the Pentacam anterior segment analysis system of corneal topography to guide RGPCL wearing.Methods In this retrospective analysis, 27 cases(50 eyes) were divided into high refractive error group and keratoconus group according to the refractive status and the corneal shape through Pentacam anterior segment analysis system. The best corrected visual acuity of two groups were observed before and 1 week, 6 months and 1 year after wearing RGPCL . The changes of corneal curvature, corneal thickness, anterior corneal surface height and posterior corneal surface height of children with keratoconus were also observed before and 1 year after RGPCL wearing.Results After 1 week, 6 months and 1 year wearing RGPCL, the changes of BCVA were statistically significant in high refractive error group and keratoconus group, compared with before treatment(P0.05). By using Pentacam three-dimensional anterior segment analysis system to observe the changes of corneal parameters in keratoconus group, the maximum curvature of the anterior surface of cornea decreased significantly after 1 years of treatment(P0.05) .Conclusions Wearing RGPCL is safe and effective for children with high refractive error and keratoconus. Pentacam anterior segment analysis system can accurately detect changes in keratoconus corneal morphology in children.
