目的:探讨时间空间相关成像技术(STIC)、二维超声心动图(2DE)及M型超声心动图在胎儿左心功能测量中的价值。方法:分别应用STIC、2DE双平面Simpson法及M型超声心动图Teichholz法测定48例胎龄为22~32周的正常胎儿心脏左室舒张末期容积(LVEDV)、收缩末期容积(LVESV)、射血分数(LVEF)及功能指标,并分析其测量结果。结果:STIC法与2DE双平面Simpson法所测得LVEDV及LVESV比较,差异均无统计学意义(P>0.05),并均有显著相关性(r=0.976、P<0.001;r=0.915,P<0.001);STIC与M型超声心动图所测得LVEDV比较,差异有统计学意义(P<0.05),所测得LVESV比较差异无统计学意义(P>0.05);M型超声心动图与2DE双平面Simpson法所测得LVEDV及LVESV比较,差异均有统计学意义(P<0.05)。3种方法所测LVEF值比较,差异均无统计学意义(P>0.05),其中STIC与2DE双平面Simpson法相关性较好(r=0.709,P<0.001),STIC法与M型超声心动图有相关性,但相关性很低(r=0.399,P<0.05)。结论:STIC可精确测量胎儿左室容积,评估胎儿左心收缩功能,与2DE双平面Simpson法所测值接近,比M型超声心动图更准确,有很好的临床应用前景。
Objective: To investigate the value of time-space-related imaging (STIC), two-dimensional echocardiography (2DE) and M-mode echocardiography in the measurement of fetal left ventricular function. Methods: Left ventricular end-diastolic volume (LVEDV), end-systolic volume (LVESV) and ejection fraction (LVESV) were measured in 48 normal fetuses with gestational age of 22-32 weeks by STIC and 2DE biplane Simpson method and M- (LVEF) and functional indicators, and analyze the measurement results. Results: There was no significant difference in LVEDV and LVESV between the STIC and 2DE biplane Simpson (P> 0.05), and there was a significant correlation between them (r = 0.976, P <0.001; r = 0.915, P (P <0.05). There was no significant difference in LVESV between STIC and M-mode echocardiography (P> 0.05); M-mode echocardiography and 2DE biplane Simpson measured LVEDV and LVESV, the differences were statistically significant (P <0.05). There was no significant difference in LVEF between the three methods (P> 0.05). STIC was correlated with 2DE biplane Simpson method (r = 0.709, P <0.001) There was a correlation but poor correlation (r = 0.399, P <0.05). Conclusion: STIC can accurately measure fetal left ventricular volume and evaluate fetal left ventricular systolic function, which is close to the 2DE bi-plane Simpson method. It is more accurate than M-mode echocardiography and has a good clinical application prospect.
