目的分析梅毒血清固定患者外周血CD4~+CD25~+Foxp3~+调节性T细胞(Treg细胞)数量及其功能的抑制情况,探讨细胞免疫功能与梅毒血清固定的关系。方法收集梅毒经规范治疗后出现血清固定患者30例及血清转阴对照患者30例的外周抗凝静脉血,应用流式细胞术分析两组患者外周血Treg细胞百分率,应用real-time PCR检测两组外周血单个核细胞Foxp3基因的表达情况,在自体APC存在的情况下检测两组Treg细胞体外抑制功能。结果梅毒血清固定组患者外周血Treg细胞占CD4~+T细胞比例(中位数:4.56%,25%~75%:3.34%~5.90%)明显高于转阴对照组(中位数:2.89%,25%~75%:2.05%~3.67%),两组相比差异有统计学意义(U=115,P=0.002<0.05);且Treg内转录因子Foxp3的表达量(17.03±2.51)也明显高于转阴对照组(7.65±0.68),两组Foxp3基因相对含量差异有统计学意义(t=8.821,P=0.043<0.05);固定组Treg细胞抑制百分率(中位数:82.35%,25%~75%:65.87%~96.60%)与对照组Treg细胞抑制百分率(中位数:85.95%,25%~75%:72.85%~96.35%)的差异无统计学意义(U=203,P=1.00>0.05)。结论梅毒血清固定者外周血Treg细胞数量增高引起的机体细胞免疫抑制可能是造成血清固定的重要原因之一。
Objective To analyze the suppression of CD4 ~ + CD25 ~ + Foxp3 ~ + regulatory T cells (Treg cells) and its function in peripheral blood of patients with syphilis and to investigate the relationship between cellular immune function and serum syphilis. Methods Peripheral blood anticoagulation venous blood was collected from 30 patients with serum-fixed patients and 30 patients with serum-negative control after standardized treatment of syphilis. The percentage of Treg cells in peripheral blood was analyzed by flow cytometry. Real-time PCR was used to detect two The expression of Foxp3 in peripheral blood mononuclear cells was detected in vitro, and the inhibitory function of Treg cells in vitro was detected in the presence of autologous APC. Results The percentage of CD4 + T cells in the peripheral blood of patients with syphilis serum fixation (median 4.56%, 25% -75%: 3.34% -5.90%) was significantly higher than that of the negative control group (median: 2.89 (P = 0.002 <0.05). The expression of Foxp3 in Treg (17.03 ± 2.51) was significantly higher than that in Treg (T = 8.821, P = 0.043 <0.05). The percentage of Treg cells in the fixed group (median: 82.35%) was significantly higher than that in the negative control group (7.65 ± 0.68), the relative content of Foxp3 in the two groups was statistically significant , 25% -75%: 65.87% -96.60%) and the control group, the percentage of Treg cell inhibition was not statistically significant (median: 85.95%, 25% -75%: 72.85% -96.35% , P = 1.00> 0.05). Conclusion The immunosuppression caused by the increase of the number of Treg cells in the peripheral blood of syphilis sera may be one of the important causes of sera fixation.
