目的 :探讨血浆转化生长因子 β1 (TGFβ1 )与其他肝纤维化指标的关系。方法 :采用双抗体夹心 EL ISA法测定 71例慢性肝病患者血浆中 TGFβ1 水平 ,并采用 RIA法检测血清透明质酸 (HA)、层粘连蛋白 (L N)、 型前胶原蛋白 (PC )、 型胶原蛋白 (Co )水平。结果 :慢性肝病患者血浆中 TGFβ1 水平显著高于正常血浆对照组 (P<0 .0 1 ) ,TGFβ1 随炎症和纤维化进展而呈逐渐升高趋势 (r=0 .4 6 2 ,P<0 .0 5 ) ,提示 TGFβ1 升高与肝病程度有较好相关。不同期肝病组之间血浆 TGFβ1 检测水平差异有显著意义 (P<0 .0 1 ,P<0 .0 5 )。慢性肝病患者中 HA、Co 、L N、PC 水平随炎症和纤维化进展而呈逐渐升高趋势 (P<0 .0 1 ) ,以 HA最明显。不同期肝病组之间两两比较差异有显著意义 (P<0 .0 1或 P<0 .0 5 )。对慢性肝病患者血浆 TGFβ1 与 HA、Co 、L N、PC 直线相关分析表明除慢性轻度组血清 PC 外 ,其他各组血浆 TGFβ1 与 HA、Co 、L N、PC 呈明显正相关 (P<0 .0 5或 P<0 .0 1 )。结论 :临床标本采用血浆更能反映肝纤维化患者 TGFβ1 水平 ,血浆 TGFβ1 检测如与以 HA为代表的细胞外间质 (ECM)等联合检测应用 ,动态观察肝纤维化进程 ,可以为临床肝纤维化检测提供非创伤性的早期诊断依据
Objective: To investigate the relationship between transforming growth factor β1 (TGFβ1) and other indicators of liver fibrosis. Methods: The plasma level of TGFβ1 in 71 patients with chronic liver disease was measured by double-antibody sandwich ELISA, and the levels of serum hyaluronic acid (HA), laminin (LN), pre-procollagen (PC) Protein (Co) levels. Results: The plasma level of TGFβ1 in patients with chronic liver disease was significantly higher than that in normal plasma (P <0.01). TGFβ1 increased gradually with the progression of inflammation and fibrosis (r = 0.4626, P <0 .0 5), suggesting that elevated TGFβ1 is associated with the degree of liver disease. The levels of plasma TGFβ1 in different stages of liver disease were significantly different (P <0.01, P <0.05). The levels of HA, Co, LN and PC in patients with chronic liver disease increased gradually with the progress of inflammation and fibrosis (P <0.01), and the most obvious was HA. There was significant difference between the two groups in different stages of liver disease (P <0.01 or P <0.05). The linear correlation analysis of plasma TGFβ1 with HA, Co, LN and PC in patients with chronic liver disease showed that there was a significant positive correlation between plasma TGFβ1 and HA, Co, LN and PC in all other groups except chronic mild serum PC (P <0. 0 5 or P <0 .0 1). Conclusion: Plasma samples can reflect the level of TGFβ1 in patients with liver fibrosis. The detection of plasma TGFβ1, such as combined with extracellular matrix (ECM) such as HA, can dynamically observe the progress of hepatic fibrosis, Assay provides a non-invasive basis for early diagnosis
