目的探讨血培养三级报告对临床诊疗菌血症的指导和应用价值。方法对我院2015年1月至9月237份阳性血液标本进行三级报告,分析一级报告时间和准确性,比较直接药敏(二级报告)与仪器法(三级报告)药敏结果的二者符合率。结果 237份血培养阳性标本,共检出革兰阳性菌105例,革兰阴性菌126列,真菌6例。一级报告时间平均为29.5h,准确率为97.89%(232/237)。革兰阳性葡萄球菌直接药敏与仪器法药敏的平均符合率为93.45%,严重错误率为0.11%,重大错误率为1.48%,一般错误率为4.97%,革兰阴性菌直接药敏与仪器法药敏的平均符合率为90.87%,严重错误率为0.32%,重大错误率为2.38%,一般错误率为6.43%,总体符合率为91.92%。结论血培养阳性标本三级报告的建立,为临床诊疗提供了准确和快速的结果,具有较大的临床应用价值。
Objective To investigate the guidance and application value of blood culture tertiary report on bacteremia in clinical diagnosis and treatment. Methods A total of 237 positive blood samples were collected from January 2015 to September 2015 in our hospital for tertiary reporting. The time and accuracy of primary reporting were analyzed. The results of direct susceptibility testing (secondary report) and instrumental method (tertiary report) were compared. The two match rate. Results A total of 237 blood culture positive samples were detected, including 105 Gram-positive bacteria, 126 Gram-negative bacteria and 6 fungi. The first report time averaged 29.5h with an accuracy rate of 97.89% (232/237). The average coincidence rate of gram-positive staphylococcus aureus with the drug-sensitive method was 93.45%, the serious error rate was 0.11%, the major error rate was 1.48% and the general error rate was 4.97% The average coincidence rate of instrument sensitivity was 90.87%, the serious error rate was 0.32%, the major error rate was 2.38%, the general error rate was 6.43% and the overall coincidence rate was 91.92%. Conclusion The establishment of the third-grade report of positive blood culture samples provided accurate and rapid results for clinical diagnosis and treatment, which has great clinical value.
