超微创磁锚定视觉诊断机器人的初步试制研制

  目的:微创是当代外科发展的潮流,经脐单孔腹腔镜技术(LESS)将微创外科引入新的阶段,多个器械共同置入造成戳卡直径大同时相互之间存在干扰,此外,腹腔镜只具备探查功能不能进行诊断,功能略显单薄。前期研究表明磁锚定视频器械在使用过程中就可以不占用戳卡,其可减少LESS的戳卡直径以及增强操作性极有意义。此外用于肝功尺检查的ICG会在肝脏肿瘤组织内沉积,在790-810nm近红外光激发下可发出830nm荧光。本研究试图将磁锚定视频器械与近红外荧光显影技术相结合,并采用微型化设计理念研制出直径10mm的磁锚定超微创视频诊断机器人,利用此机器人一方面可减小LESS手术的切口大小同时增强其操作性,另一方面可进行肝脏浅表微小肿瘤的诊断,提高微创手术治疗的精准性。方法:首先进行骨架研制,在仿真分析的基础上,通过微型化设计,利用微型CMOS摄像头、高色温LED光源、钕铁硼磁体完成直径10mm的超微创磁锚定视频器械的研制,通过与传统腔镜器械配合进行减戳卡LC手术检验其可行性并加以改进;随后进行视觉诊断模块的研制,视觉诊断模块由808nm光源与微型激光输出头构成,其中微型激光输出头研制是核心,需能满足808nm近红外光在超微创磁锚定视频器械中应用的要求;最后进行集成,将二者结合完成超微创磁锚定视觉诊断机器人的研制。结果:所研制超微创磁锚定视频器械直径恰控制在10mm,可通过10mm传统腔镜戳卡,所提供图像质量能够满足LESS手术条件需求,其与传统腔镜手术器械配合能够完成基于两个传统腔镜戳卡的LC手术,表明其可作为超微创磁锚定视觉诊断机器人的骨架。所研制视觉诊断模块中的8mm微型激光输出头可实现90°输出角并在7cm高度产生直径20mm光斑,从而满足808nm近红外光在超微创磁锚定视频器械中应用的要求。结论:本研究中完成了超微创磁锚定视觉诊断机器人的骨架及视觉诊断模块的研制,结构表明能够满足超微创磁锚定视觉诊断机器人在微创性及功能性方面的设定,后续研究中将二者集成即可完成超微创磁锚定视觉诊断机器人的研制,有助于提高LESS手术的微创性、操作性及精准性。
