经皮与经肌间隙椎弓根螺钉内固定治疗胸腰椎骨折的疗效比较

  目的:探讨新型经皮椎弓根螺钉(Zina)内固定治疗胸腰椎骨折的临床疗效.方法:2013年9月~2014年2月采用三友Zina脊柱微创内固定系统经皮椎弓根螺钉内固定治疗无神经损伤、Loadsharing评分≤6分的胸腰椎A型骨折患者30例(经皮组),与同期行经肌间隙椎弓根螺钉内固定治疗的30例胸腰椎骨折患者(经肌间隙组)进行比较,观察两组的临床与影像学指标.结果:两组的手术切口长度、手术时间、术中出血量及术后第1天切口疼痛经皮组分别是(7.35±0.76)cm、(95.7±14.2)min、(53.7±19.9)ml及(3.7±1.3)分;经肌间隙组分别是(9.80±1.92)cm、(82.3±16.4)min、(122.0±28.2)ml及(5.8±1.3)分,两组间的差异有统计学意义(P<0.05).两组的术前、术后即刻及末次随访时的伤椎前缘高度和矢状面Cobb角经皮组分别是(60.8±12.1)%、(88.3±6.9)%、(86.5±7.2)%、(18.5±4.9)°、(4.9±2.5)°及(5.2±1.7)°;经肌间隙组是(61.8±9.9)%、(91.1±4.3)%、(91.3±4.2)%、(19.0±3.7)°、(4.7±3.5)°及(4.8±3.4)°,两组间的差异无统计学意义(P>0.05),两组术后即刻的伤椎前缘高度和矢状面Cobb角与术前相比均得到明显纠正(P<0.05),两组末次随访时的伤椎前缘高度和矢状面Cobb角与术后即刻相比均有丢失,但是差异无统计学意义(P>0.05).结论:严格掌握手术适应证的情况下,新型经皮椎弓根螺钉(Zina)内固定具有和经肌间隙开放手术相似的临床效果,同时又具有手术切口更短、术中出血更少、术后疼痛更轻等优点.
